Skip mediastinal lymph node metastasis in non‐small cell lung cancer

Abstract: We retrospectively investigated 186 non-small cell lung cancer cases with N2 disease in order to clarify the significance of skip metastasis. Of the 186 patients with N2 disease, negative N1 nodes recognized to be skip metastasis were seen in 62 patients (33%). The incidence of skip metastasis was not statistically different regarding histology, T status, or M status. The incidence of the skip metastatic site consisted of 35 cases (56%) at sites 1, 2 and 3, while 8 cases (13%) were found at sites 8 and 9. Amon… Show more

“…This relationship between the location of the primary site and the frequency of mediastinal skip nodal metastasis was also documented by other investigations [3,7,13,14]. From the viewpoint of sentinel lymph node mapping, the results of recently launched clinical trials support the anatomical concept.…”

“…3,10,11 In our results, 37% of the patients with skip metastases were 5-year survivors whereas 22% of those without skip metastases survived Ͼ 5 years, and there was a significant difference between the 2 groups. The tendency remained when the patients were limited to those with the metastasis of a single N2 level.…”

“…9 When no involved hilar lymph nodes were observed but the mediastinal lymph nodes were involved by carcinoma, the tumor was regarded as having skip metastasis. 10,11 Histologic typing was performed according to the World Health Organization classification. 12 The follow-up period was Ͼ 5 years for all patients.…”
