Skin sparing mastectomy: Technique and suggested methods of reconstruction

Abstract: Skin Sparing mastectomy through a circum-areolar incision has proven to be a safe and feasible option for the management of breast cancer in Egyptian women, offering them adequate oncologic control and optimum cosmetic outcome through preservation of the skin envelope of the breast when ever indicated. Our patients can benefit from safe surgery and have good cosmetic outcomeby applying different reconstructive techniques.

“…Breast conservation therapy and mastectomy are both viable options with similar survival benefits, but the individual preference of patients should be integrated into treatment decisions when deciding between the two 34 . Oncoplastic-augmentation-mastopexy is an oncoplastic surgical option developed for patients with previous breast augmentation or small breast volume who choose to preserve their breast size and avoid mastectomy by opting for BCT, while also avoiding a second operation by going directly to implants 28–30,35,36 …”

“…Well-established techniques have been described for large- and medium-sized breasts; however, oncoplastic surgical treatment options for patients with a low breast volume are limited. Literature describing the results of breast augmentation in conservation therapy is also limited 8,28–30 . History of breast augmentation, large tumor size to be removed, or small breast size poses a challenge for breast conservation and reconstruction management due to breast volume that is too small to achieve adequate breast shape with soft tissue–only oncoplastic techniques 11,16,17,31 .…”

“…Incisions for NSM include inframammary, extended inframammary, lateral inframammary, horizontal pericentral (extending from the lateral aspect of the areola), and superior areolar crescent also known as the modified nipple sparing (Figure 1). Our personal preference is the inframammary approach, which has proven to be both effective and safe (36,37). In our opinion, the inframammary incision leads to the most pleasing aesthetic outcome with little or no visible scar with the breast in the upright position.…”
