Skin pre‐ablation and laser assisted microjet injection for deep tissue penetration

Abstract: Fractional-rotational ablation increases number of micro-holes in a unit area, enabling fast reepithelialization and high drug delivery efficiency. Optimization of system parameters such as ablation time, number of ablations, and injection time will eventually ensure a macromolecule delivery technique with the potential to include vaccines, insulins, and growth hormones, all of which require deeper penetration into the skin. “…Microjet velocities of 23.0 to 50.6 m/s provided effective delivery of epidermal growth factor and human growth hormone in porcine skin, as determined by gene expression of keratinocyte laminin and fibroblast elastin, respectively. A subsequent refinement of the procedure has involved a pre-ablation step prior to microjet generation, both achieved by the same laser [120]. The advantage is that pre-ablation is targeted to finite areas, with the microjet then bombarding those defined areas.…”

“…The jet injection systems that have been trialled for the delivery of dental local anaesthetic have all been driven by an uncontrollable energy source, such as the release of a compressed spring [8], [12], [16]. Recently, focus in the field of jet injection has moved to controllable devices driven by electric motors [17], piezoelectric actuators [18], laserinduced cavitation [19], [20], or controlled pneumatic sources [21]. These injectors are able to develop a wide range of precisely-controlled fluid pressures and are capable of adjusting pressure dynamically during the injection.…”

“…It is a minimally invasive surgical technique, which can deliver various molecules into the subdermis using a high‐speed liquid jet. This technology is expected to have various potentials including the ability to increase penetration efficiency by pre‐ablating the epidermis with fractional lasers with rotating beams . Initially, this technique was studied to replace traditional needles in situations that require the repeated delivery of macromolecules, such as growth hormone and insulin .…”
