Site of clear corneal incision in cataract surgery and its effects on surgically induced astigmatism

Abstract: Clear corneal incision (CCI) is a commonly used surgical approach in cataract surgery. In this prospective study, we evaluated the effect of CCI site on surgically induced astigmatism (SIA) and other postoperative astigmatic changes. CCIs were constructed based on the steep meridian of the total corneal refractive power in the 4.0-mm-zone (TCRP4.0), and patients were divided into four groups: temporal, superotemporal, superonasal, and superior according to the site of the incision. TCRP4.0 analysis demonstrate… Show more

“…A meta-analysis study reported that SMILE procedures did not have obvious superiority over femtosecond-assisted laser in situ keratomileusis, after revealing similar and acceptable objective parameters with SMILE, potentially resulting in milder subjective symptoms [13]. Further, we used the AKP analysis method [12] to evaluate the SIA changes and found that AKP(+ 0) showed statistically signi cant changes in both groups, and uctuation in corneal surface irregularity outcomes masked changes of polar SIA values after SMILE surgery. This meant that the corneal wound healing processes in uenced corneal surface irregularity and corneal polar SIA values during various postoperative visits.…”

“…All postoperative follow-up visits included the assessment of UDVA, IOP and corneal topography (TMS-4N, TOMEY, Erlangen, Germany). Polar SIA was analysed by the polar value analysis method [12].…”

“…With an effective sealant, corneal surgery would not be restricted to complex incisional architecture. 37 , 38 The availability of pectin film may permit larger or secondary incisions when necessary without the increased risk of perioperative morbidity. Although pectin's biodurability on the cornea and compatibility with tears is unknown, previous studies of the pleural surface of the murine lung suggest a pectin half-life of 7 days.…”

“…Astigmatic polar value of net astigmatism (AKP) analysis methods [19] were used to analyze astigmatism changes after surgery. All keratometric values were converted to a plus power (@) net cylinder format with the magnitude of keratometric astigmatism in diopters, and the direction in degrees following the steepest keratometric meridian axis.…”
