Sitagliptin's renoprotective effect in a diabetic nephropathy model in rats: The potential role of PI3K/AKT pathway

Abstract: Management of diabetic nephropathy (DN) is far from satisfactory. There is a rising role of the involvement of phosphatidylinositol 3-kinase (PI3K)/protein kinase B (AKT) pathway in the pathogenesis of DN. This study aimed at investigating the renoprotective effects of PI3K/AKT pathway via sitagliptin in a rat model of DN. Thirty-two male Wistar rats were divided into four groups (eight rats each): (I) control, (II) sitagliptin, (III) DN, and (IV) DN + sitagliptin. Fasting blood glucose (FBG), kidney index, an… Show more

“…Additionally, 16 weeks of treatment with low-or high-dose sitagliptin dramatically reduced serum urea and creatinine, urine albumin (microalbuminuria) levels in Wistar rats. The same sitagliptin therapy also led to improvements in glomerular lesions 48 .…”

“…While in treated groups (insulin or di-PEGylated insulin), these pathological features were somewhat attenuated (such as normal glomeruli and narrow irregular urinary space), particularly di-PEGylated insulin. For PAS staining, thicker glomerular basement membrane and incomplete brush border were noticed in mice treated with PBS or insulin 31 . While in the di-PEGylated insulin group, most renal tubules were presented with completed brush border and glomerulus were seen with preserved glomerular basement membrane.…”

“…As a negative control, representative H&E images of PBS-treated mice showed classical diabetic pathology, e.g. , shrunken glomeruli, wide irregular urinary space, and vacuolated renal tubular cells 31 . While in treated groups (insulin or di-PEGylated insulin), these pathological features were somewhat attenuated (such as normal glomeruli and narrow irregular urinary space), particularly di-PEGylated insulin.…”

“…Sitagliptin not only lowers blood glucose levels but also exhibits anti-inflammatory (14,15) and immunomodulatory effects (26). Considering the elevated levels of inflammatory factors in COVID-19 (27,28), sitagliptin could help mitigate complications and the disease's severity. It activates AMPK through liver (32).…”
