‘Sister, I am going crazy, help me’: psychodynamic‐oriented care in psychotic patients in inpatient treatment

Abstract: A common German expression for 'crazy' means literally 'not sealed'. In psychoses, the boundaries of the ego are brittle and dissolved, the pores are open. Fragmentations occur, fusions of undefined self and object represenations are impending. The self is projected and introjected, always struggling for a sealed-up, though sufficiently permeable, space. These psychodynamics form a characteristic feature of professional psychiatric care. Providing a sealing up and cohesive relation as a basis for the return in… Show more

“…The importance of communication between psychiatric nurses is consistently documented in the literature (Barker, 1999;Dunwell & Hanson, 1998;Horsfall & Stuhlmiller, 2000;Jackson & Stevenson, 2000;O'Brien, 1999;Teising, 2000). “…The acknowledged role of nurses in establishing and maintaining a therapeutic relationship with clients (Barker, 1999;Dunwell & Hanson, 1998;Horsfall & Stuhlmiller, 2000;Jackson & Stevenson, 2000;O'Brien, 1999;Teising, 2000) suggests they have a pivotal role in encouraging and facilitating consumer and carer involvement. It might therefore be considered surprising that an extensive review of the literature did not reveal any research addressing the attitudes of psychiatric nurses towards consumer and carer participation.…”

“…The nurse and the patient verbalize their experiences and examine, analyse and clarify their meanings in order to understand one another (Tilley 1999). Teising (2000) has described psychodynamic‐orientated care in psychotic patients in inpatient treatment. Teising (2000) points out that one of the essential tasks of psychiatric nursing is to provide cohesive relationships as a means of returning to a non‐psychotic world.…”

Teising (2000) points out that one of the essential tasks of psychiatric nursing is to provide cohesive relationships as a means of returning to a non‐psychotic world. Thus, the nurse has the task of containing the raw material of the patient's psychosis through a process of empathy.…”
