Single-incision transabdominal preperitoneal and totally extraperitoneal repair for inguinal hernia: Early experience from a single center in Asia

Abstract: Single-incision laparoscopic inguinal hernia is feasible in both TEP and TAPP approaches. The procedure should be performed by laparoscopic surgeons with a high level of experience in single-incision surgery. Further randomized trials should be performed to evaluate the full potential and clinical application of single-incision TAPP and TEP.

“…In this series, none of the patient was found to have umbilical hernia upon follow-up. Similar result with no occurrence of umbilical hernia was also shown in our experience with 15 elective cases of single incision hernia repair performed in 2010 to 2011 [2]. In our unit, polydioxanone suture (PDS TM II, Ethicon) was used to close the fascial defect.…”

“…The first single incision laparoscopic surgery (SILS) was performed in 2009 in our unit. The initial results of SILS in elective hernia repair in our center were comparable to those of conventional laparoscopic approaches [2].…”

“…Since single incision laparoscopic surgery has gained in popularity, single incision TAPP has also been reported (4,5). It has an impact on the cosmetic result; however, it needs a 2 to 2.5 cm incision at the navel, which should be associated with the risk of incisional hernia and other wound-related complications.…”

“…This challenge was attempted also for laparoscopic transabdominal preperitoneal herniorrhaphy (TAPP), and thus both single incision TAPP (4,5) and needlescopic TAPP (2,6) have been developed . In the literature reporting on clinical outcomes, needlescopic TAPP procedures were performed by 3 mm and 5 mm instruments, whereas we developed instruments to perform TAPP via even smaller wounds, 2 mm, 5 mm and 2 mm (TAPP-252), in a reproducible manner.…”
