Simultaneous small bowel and colon obstruction due to splenosis. A case report and review of literature

Abstract: Highlights Splenosis is an autotransplantation of splenic tissues throughout the body post splenic rupture or splenectomy. Symptomatic when hematological diseases have recurred. Abdominal splenosis may rarely be associated with abdominal pain and/or gastrointestinal symptoms. Further evaluation and treatment is mandatory when bowel obstruction is present. Treatment consists of resection of problematic splenic nodules.

“…Gastrointesitnal bleeding occurs when splenic tissue is located within the bowel wall and through intestinal mucosae. Bowel obstruction caused by bridges and adhesions between splenic implants in mesentery and bowel wall has been reported [ 11 , 12 ]. However, bowel obstruction due to adhesions could be triggered not by the splenosis itself, but by a previous splenectomy, making splenosis an incidental finding after all [ 13 ].…”

“…Most patients with splenosis are asymptomatic. It is usually found accidentally, during physical or imaging examination, autopsy or abdominal surgery for another reason [ 7 , 9 , 10 ]. That was the case with our patient who had the splenosis discovered during diagnosis for nephrolithiasis.…”

“…That was the case with our patient who had the splenosis discovered during diagnosis for nephrolithiasis. The symptoms may include nonspecific abdominal pain, palpable abdominal mass, gastrointestinal or intraabdominal hemorrhage, bowel obstruction or hydronephrosis [ 2 , 3 , 10 ]. Spleen implants remain functional, which can have positive and negative implications.…”
