Sigmoid Adenocarcinoma with Renal Metastasis

Abstract: We report a case of a 75-year-old man submitted to a rectosigmoidectomy and partial cystectomy because of a sigmoid cancer and colovesical fistula. Seven months later and after four cycles of adjuvant chemotherapy, a lesion was detected in the kidney. Histology revealed tubular adenocarcinoma, which meant sigmoid cancer metastasis. Kidney metastases are very rare in colorectal cancer (CRC), but may be generally associated with an unfavorable prognosis. Thus, patients with metastatic CRC and kidney tumors are a… Show more

“…A biopsy was performed to rule out a second primary renocellular cancer. Systemic chemotherapy is the treatment of choice for CRC renal metastases and, with the exception of rare emergencies or cases with resectable disease, nephrectomy is not commonly used in the management of CRC renal metastases (14,23). Other relative indications for surgery are tumors with perirenal hematoma and risk of rupture, as well as symptomatic patients (9).…”

“…Colorectal tumors58–63 represent 4.7% to 22% of cases with renal metastases 4,6,27. In an autopsy series, metastases to the kidney from a colorectal primary was found in only 2.7% of cases and was frequently associated with widespread metastases 60. Pancreatic tumors rarely metastasize to the kidney with an incidence of 2.7%6; the route is usually hematogenous but direct invasion to the adjacent left kidney may also occur 64,65.…”

“…7,8 However, systemic chemotherapy is the treatment of choice for metastatic colorectal carcinoma and nephrectomy is not commonly used in the management of renal metastases from colorectal cancer. 6,8,9 The metastatic pathway in such cases is usually by haematogenous spread. Therefore, this case is quite remarkable and very rare as the spread occurred by direct intraureteric growth.…”
