Side-to-Side Jejunoileal Bypass Induces Better Glucose-Lowering Effect than End-to-Side Jejunoileal Bypass on Nonobese Diabetic Rats

Abstract: This study shows that side-to-side jejunoileal bypass induced better glucose-lowering effects than end-to-side jejunoileal bypass and proximal small bowel resection, and intestinal bypass surgery that retained shorter distal small bowel yielded better antidiabetic effects.

“…SSJIBL: This procedure bypasses approximately 60% of the length of the entire small bowel, with details of the procedure used in a previous study ( Duan et al, 2015 ). Briefly, a point 30 cm proximal to the ileocecal valve and another point 6 cm distal to the ligament of Treitz were used as the reference points.…”

“…An increasing number of researchers believe that the small intestine plays a key role in regulating glucose metabolism ( Duca et al, 2021 ). Previously, our team designed a side-to-side jejunoileal bypass plus proximal loop ligation (SSJIBL), which bypasses the proximal small intestine and can significantly improve glucose metabolism with elevated total serum BA (TBA) levels ( Duan et al, 2015 ). However, the mechanism of elevated BA levels and the regulation of glucose metabolism are unclear.…”

“…Similarly, performing anastomoses is endoscopically challenging. For the treatment of obesity, type II diabetes and malignant obstructions, a completely endoscopic method of creating a durable reliable bowel bypass is desirable [45][46][47].…”

“…Un mecanismo, distinto al mediado por el GLP-1 y otras enterohormonas, es el aumento de la expresión de los productos de los genes REG después de la modificación anatómica. Estos productos favorecen la regeneración pancreática, específicamente Reg3 gamma, que se ha asociado con las concentraciones de insulina y péptido C en pacientes que reaccionaron favorablemente a la derivación gástrica en Y de Roux por laparoscopia 56,57 .…”
