Background:Prenatal screening deals with the detection of structural and functional abnormalities in the fetus. Health care providers can minimize unintended pregnancy outcomes by providing proper counseling and performing prenatal screening. The purpose of the present review study is to investigate factors affecting improved prenatal screening.Methods:The present study is a narrative review searching public databases such as Google Scholar and specialized databases such as Pubmed, Magiran, Scientific Information Database, Elsevier, Ovid and Science Direct as well. Using the keywords “prenatal screening”, “fetus health” and “prenatal counseling”, 70 relevant articles published from 1994 to 2014 were selected. After reviewing the abstracts, the full data from 26 articles were ultimately used for writing the present review study.Results:Three general themes emerged from reviewing the studies: health care providers’ skills, clients’ characteristics and ethical considerations, which were the main factors affecting improved prenatal screening.Conclusion:Prenatal screening can be successful if performed by a trained and experienced expert through techniques suitable for the mother’s age. Also simultaneously providing proper counseling and giving a full description of the risks and benefits of the procedures for clients is recommended.

En el Instituto de Medicina Fetal y Genética Humana de São Paulo se ofrecen, a las gestantes que tienen un riesgo aumentado para anomalías cromosómicas, diferentes técnicas, entre ellas, la Biopsia de Vellosidad Corial Transabdominal (BVCTA) y la Amniocentesis Precoz (AP). El objetivo del presente estudio es realizar una comparación de la frecuencia de pérdidas fetales y anomalías congénitas presentadas en cada uno de los procedimientos, ambos realizados por los mismos operadores, en la misma edad gestacional (12-14 6/7 semanas) y bajo un abordaje transabdominal. Fueron analizadas retrospectivamente 432 AP y 418 BVCTA. Todos los procedimientos de colecta fueron monitorizados por ultrasonografía. La frecuencia de pérdidas fetales espontáneas fue del 4,9 % en AP y de 5,3 % en BVCTA, una diferencia no significativa. No se encontraron diferencias significativas entre los dos procedimientos al comparar las frecuencias de pérdidas en cada semana de gestación. Sangrado y pérdida de líquido amniótico fueron más frecuentes en AP que en la BVCTA. Esa diferencia fue significativa en el caso de la pérdida de líquido amniótico. En algunoscasos este hallazgo se relacionó con pérdida fetal. La incidencia de prematuridad y bajo peso al nacimiento no difirió significativamente entre los dos procedimientos. La mayor frecuencia de problemas respiratorios registrada en AP no fue significativa en comparación con BVCTA. No se observó diferencia significativa en la incidencia de anomalías músculo-esqueléticas. La amniocentesis después de catorce semanas presenta un bajo riesgo de pérdida fetal o anomalías congénitas. La BVCTA, debe ser realizada alrededor de la semana doce de gestación.

ÖzetAmaç: Kliniğimizde son 5 yılda amniosentez yapılan hastalarda işleme bağ-lı fetal kayıp riskinin ve kötü gebelik sonuçlarının artıp artmadığını değer-lendirmek. Gereç ve Yöntem: Bu retrospektif çalışma Kahramanmaraş Sütçü İmam Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum kliniğinde 2011 Ocak ile 2015 Temmuz tarihleri arasında amniosentez yapılan 387 hastanın dosyalarından ve genetik merkezin kayıtlarından incelenerek yapıldı. Ayrıca kliniğimize kontrole gelen ve amniosentez yapılmayan düşük riskli 250 hasta kontrol grubu olarak belirlendi. Bulgular: Çalışma süresince amniosentez endikasyonu olan 688 hasta mevcuttu. Bunlardan 387 hastaya amniosentez yapıldı. %43.8 amniosentez reddetme oranı mevcuttu. En sık amniosentez endikasyonu anormal Down sendromu tarama testi iken (%57.6), 2. sık-lıkla ileri maternal yaş (%22.5) görülmektedir. Amniosentez yapılan hastaların 24'ünde (%6.2) kromozomal anormallik mevcut olup bunlar içerisinde en sık Down sendromu izlendi (%54). Amniosentez sonrası fetal kayıp 2 hastada (%0.5) görülmüştür. Toplamda kötü gebelik sonuçlarına bakıldığında amniosentez yapılanlarda 8, kontrol grubunda 5 hastada kötü gebelik sonuçları izlenmiş olup gruplar arasında istatisitksel anlamlı fark yoktu (p=0.263). Tartışma: Amniosentez günümüzde sıklıkla kullanılan invaziv prenatal tanı testidir. İşleme bağlı olarak gebelik komplikasyonlarında bir artış görülmemekte-dir. Amniosentez öncesinde hastalara mutlaka yapılacak işlem ve sonuçları hakkında detaylı danışmanlık verilmelidir.
