Background and aims. Lysozyme and lactoferrin are salivary proteins which play an important role in innate defense mechanisms against bacteria. This study investigated the association of salivary lysozyme and lactoferrin concentrations with early childhood caries (ECC).Materials and methods. This study was carried out on 42 healthy children (age range, 36 to 71 months), of whom 21 were caries free (CF) and 21 had ECC. Disposable needle-less syringes were used to collect unstimulated saliva from buccal and labial vestibules. Fifteen children who had ECC were treated completely and their saliva was collected in the same way for the second time, three months after treatment. Lysozyme and lactoferrin concentrations were measured and recorded by the ELISA method. The intergroup comparisons were carried out using chi-square, Student’s t-test and Wilcoxon signed ranked test. A P-value less than 0.05 was considered as statistically significant.Results. The mean concentration of lysozyme was significantly higher in CF group compared with that of ECC group (P = 0.04). Although the mean concentration of lactoferrin in ECC group was higher in comparison with ECC group, the difference was not statistically significant (P = 0.06). After dental treatment, the mean concentrations of lysozyme and lactoferrin did not change in comparison with their concentrations before treatment.Conclusion. ECC may have a relationship with lower concentrations of unstimulated salivary lactoferrin and lysozyme and reduced amounts of these two salivary proteins may be a risk factor for dental caries in children.

Saliva is a biological fluid of complex composition that facilitates the interactions between host cells, oral microorganisms and immune modulators with the main purpose of maintaining oral health status. Within the salivary components, proteins are fundamental, because they are responsible for the main functions of saliva. Proteomic studies contributed to identify and characterize between 2,000 and 2,600 different proteins and peptides in their composition. Among them, a high proportion is glycoproteins, such as mucins, proteins rich in proline and immunoglobulins, agglutinins, lactoferrin, cystatins and lysozyme. In addition, saliva possesses peptides of potent antimicrobial activity and protection to mineral tissues, such as cathelicidin LL-37, α and β defensins, histatins and statins. In particular, in recent years the selection of biomarkers of oral diseases such as dental caries, periodontal diseases and oral cancer has been focused, considering the simple, economical and non-invasive obtaining of this fluid, which has driven the development of technologies for the detection of biomarkers with high sensitivity and specificity for a wide variety of oral diseases. The study of the components of saliva and its potential biomarkers makes this fluid a powerful tool for diagnosis and detection of diseases, as well as to evaluate the evolution of therapeutic treatments, because it reflects the physiological and pathological state of the body.KEY WORDS: salivary proteins and peptides, biomarkers, dental caries, periodontal diseases, oral cancer ResumenLa saliva es un líquido biológico de composición compleja que facilita las interacciones entre las células del huésped, los microorganismos bucales y los moduladores inmunológicos con el principal propósito de mantener el estado de salud bucal. Dentro de los componentes salivales, las proteínas son fundamentales por ser responsables de las principales funciones de la saliva. Los estudios proteómicos contribuyeron a identificar y caracterizar entre 2.000 a 2.600 proteínas y péptidos diferentes en su composición. Entre ellas, una alta proporción son glucoproteínas, como las mucinas, proteínas ricas en prolina e inmunoglobulinas, aglutininas, lactoferrina, cistatinas y lisozima. Además la saliva posee péptidos de potente actividad antimicrobiana y de protección a los tejidos minerales, como la catelicidina LL-37, α y β defensinas, histatinas y estaterinas.
