Summary:Purpose: To analyze systematically hyperorality associated with epileptic seizures and its relation to the localization of epileptic activity.Methods: To identify patients with periictal hyperorality, we reviewed video-recordings of 269 patients (aged 6-59 years) who had consecutively undergone presurgical evaluations including ictal video-EEG recordings and high-resolution magnetic resonance imaging (MRI) and had had epilepsy surgery because of intractable frontal (FLE) or temporal lobe epilepsy (TLE). Periictal hyperorality was defined if patients put or unambiguously intended to put nonfood items into their mouths during or after at least one of the reviewed seizures. For the further analysis, we included only patients with periictal hyperorality. We reviewed their medical records and reexamined their ictal video-EEG recordings.Results: We identified eight patients (six women) aged 8-59 years who had hyperorality during or after seizures. Seven patients had TLE, and one patient had frontal lobe epilepsy (FLE).Three of these patients underwent right-sided surgery, whereas five patients had surgery on the left. Three patients exhibited ictal and five showed postictal hyperorality. Interictal EEG suggested bilateral interictal epileptiform discharges (IEDs) in three patients; in two other patients, no IEDs were detected. Ictal EEG suggested bilateral involvement in six cases. Patients with unilateral epileptiform activity had left TLE.Conclusions: Periictal hyperorality is a rare phenomenon occurring in 3% of the investigated epilepsy population. We suggest that periictal hyperorality is an ictal-postictal mental disturbance, an incomplete Klüver-Bucy syndrome. In most patients, bilateral seizure activity plays an important role in the pathomechanism, but it would appear that left-sided epileptic activity without contralateral involvement also can cause periictal hyperorality.

Autonomic symptoms frequently occur during epileptic seizures either as an accompaniment to other seizure symptoms or as the predominant seizure manifestation. They do not represent simple reactions to motor manifestations of seizures, but are mediated by an activation of the central autonomic network. Autonomic symptoms can be divided into cardiovascular changes, respiratory manifestations, gastrointestinal symptoms, cutaneous manifestations, pupillary symptoms, genital and sexual manifestations as well as urinary symptoms. Due to a hemispheric‐specific representation of the central autonomic network, certain autonomic symptoms may provide lateralizing and sometimes localizing information on the seizure onset zone, although some of these signs may appear as a result of discharge spreading. Autonomic symptoms indicating a seizure onset in the non‐dominant hemisphere include ictal vomiting and retching, spitting automatisms and ictal urinary urge. Autonomic symptoms range from subtle seizure manifestations which become apparent only during meticulous seizure analysis, to severe, sometimes life‐threatening events. Cardiovascular and respiratory autonomic symptoms are discussed as the mechanisms underlying sudden unexplained death in epilepsy. When autonomic symptoms represent the sole seizure manifestation, they can pose problems for differential diagnosis of various nonepileptic conditions. Finally, autonomic seizure symptoms open a unique window on the functional organization of the central autonomic network and on brain function in general. (Published with videosequences.)

The KBS was first described on the basis of experimental bilateral temporal lobectomy in monkeys. Extrapolating any syndrome from monkey to man is frought with controversy, but the 53 patients with KBS reported here are significant for the completeness of its expression and the opportunity to follow its evolution. The rarity of the human KBS is attested to because less than 200 cases have been reported in the literature. The KBS in man, presented here as a clinical entity, includes the following symptoms: (1) Increased oral activity: a strong tendency to examine all objects orally, putting objects in mouth, licking, biting, chewing, touching with lips. (2) Hypersexuality (hetero-, homo-, autosexual). (3) Hypermetamorphosis: to touch everything in sight, to attend to every visual stimulus with grasping, hugging movements. (4) Memory disorders. (5) Placidity, flattened affect and pet-like compliance with lack of aggressive affective behaviour. (6) Loss of recognition of people (7) Bulimia. The KBS in man shows little etiologic specificity and has been seen in our cases with posttraumatic encephalopathy, encephalitis, anoxia and subarachnoid hemorrhage as well as with Pick's disease, Alzheimer's disease, bilateral temporal infarction, partial or complete removal of both temporal lobes, hypoglycemia, adrenoleucodystrophy, porphyria and neuroleptic medication reported in previous studies. Since the lesions in man are not as extensive and not as consistently localized as in animal experiments, it is not surprising that all abnormal behaviour deficits seen in monkey following bitemporal lobectomy have seldom been exactly reproduced in man. The full syndrome is not usually seen. Indicating a partial KBS in man, combinations of three or more different elements must be present concerning cognitive, behavioural and emotional deficits. In most cases of human KBS the evolution demonstrates a transient profile but in few patients a partial KBS is remaining. The dynamics in the development of a KBS show a delineation between a pre-stage, full-stage, and remission stage. These stages only occur in posttraumatic cases and have not been seen in KBS of other etiology. The results of CT-investigations in 50 patients with KBS mainly show nonspecific abnormalities (brain atrophy, focal lesion, subdural hematoma, hydrocephalus) and the exact anatomic localization of the lesions necessary to produce KBS could not be demonstrated in particular cases by CT. Also no specific EEG-pattern is associated with KBS and the EEG often shows a wide range of findings.(ABSTRACT TRUNCATED AT 400 WORDS)
