Objective: While many individuals diagnosed with schizophrenia report childhood trauma, little is known about the relationship between childhood abuse and intelligence in this population. Authors hypothesized the abused individuals would show lower verbal IQ. Methods: This preliminary study compared the intelligence profile between 46 adults experiencing first-episode schizophrenic psychosis with and without a history of childhood physical or sexual abuse. Clinical assessment included the Korean Wechsler Adult Intelligence Scale (K-WAIS) and an interview assessing history of sexual and repeated physical abuse before age 16. Results: The mean verbal IQ score of patients with a history of childhood abuse (n=21) was significantly lower than that of patients without a history of childhood abuse (n=25; p < .05). However, the mean number of years of education was also significantly lower in those with a history of childhood abuse (p < .001). In fact, an analysis of covariance controlling for education revealed no group differences in intelligence. Conclusions: Childhood trauma may contribute to low verbal intelligence of firstonset patients diagnosed with schizophrenia by leading to poor educational attainment. Assessment and management of first-onset patients with childhood abuse need to be tailored for their intellectual level.

Schizophrenia is a challenging and complex psychiatric disorder. It is a chronic disorder of thought, affect, and cognition that significantly disturbs the individual's ability to function in society and develop interpersonal relationships. The clinical presentation can be extremely varied, with symptoms including delusional thinking, disorganized thoughts and speech, hallucinatory behavior, and negative symptoms (e.g., blunted affect, avolition, alogia, anhedonia). Approximately 1% of the population is affected by schizophrenia worldwide, and women may experience different symptoms, have a later age of onset, may respond to different treatments, and may be more concerned about specific side effects than men. Women with schizophrenia traditionally have been treated in the same way as men and have generally had poorer comprehensive medical care. With the introduction of many new antipsychotic medications in recent years, this review focuses on sex differences in schizophrenia, with an emphasis on differences in treatment and side effects. Additionally, it presents patient counseling issues in sexuality and health outcomes.

Childhood trauma has long been recognised as a potential cause for a range of affective mental health problems arising in adulthood. Only in recent years has the association between childhood abuse and psychosis begun to be investigated. This paper provides a critical review of the literature addressing the relationship between childhood abuse and psychosis. Implications for practitioners are discussed, including practice, policy, treatment and child protection issues. A significant proportion of people develop psychosis in adulthood following all types of childhood abuse, including people diagnosed with schizophrenia, major depressive disorders, dissociative identity disorder and post-traumatic stress disorder. Evidence suggests the possibility of a causal relationship between childhood abuse and psychosis in adulthood. Mental health nurses are ideally placed to offer help, care and support to those individuals who experience psychosis by acknowledging and listening to their life events, including experiences of childhood abuse.
