Osteoporosis is increasingly found in the elderly and senile, maintaining its enormous medical and social significance. The effect of hormones on bone metabolism is beyond doubt. However, currently the data on the effect of sex hormones on bone tissue prevails. As for the other hormones, sometimes, there are conflicting opinions. The aim of the study: Based on published data, to study the contribution of the endocrine system to the development of osteoporosis in the elderly. Materials and methods: Literature data was analyzed using the following search words: osteoporosis, bone mineral density, FSH, estrogens, testosterone, cortisol, vitamin D, IGF1 for 1998-2020 in computer databases PubMed, Scopus, Medical- Science, Elibrary, Web of Science, Ceeol. Results: Analysis of the literature showed that the increase of levels of thyroid stimulating hormone (TSH) plays an osteoprotective role; the decrease of levels of estrogen, testosterone, insulin-like growth factor 1 (IGF1) and vitamin D, as well as the increase in the levels of cortisol, parathyroid hormone and follicle-stimulating hormone (FSH) contribute to bone loss in the elderly and senile. In addition, the FSH receptor (FSHR) genotype AA rs6166 is associated with low bone mineral density, regardless of estrogen level. A polyclonal antibody with an FSHR-binding sequence against mouse β-subunit of FSH is likely to be an effective tool for reducing bone loss in mice subjected to ovariectomy. Conclusion: A comprehensive assessment of the hormonal profile in the elderly and senile is needed to identify the causes of osteoporosis and the formation of an individual program of medical diagnostic and rehabilitation measures. Currently, there are all prerequisites for the development of new diagnostic and therapeutic interventions for the correction of low bone density.

Aging is associated with generally accepted changes in brain functions, including cognitive ones. In addition, age makes its own adjustments to the work of the endocrine system. In turn, a change in the hormonal background during the aging process imprints the work of brain cells, cognitive functions, and socio-emotional functioning. Investigated, the relationship between sex hormones, cortisol, oxytocin and cognitive and socio-emotional functioning. Sex hormones are involved in neurite growth, synaptogenesis, dendritic branching, myelination, and other important mechanisms of neural plasticity. Physiological and pathological conceptualized theories suggest how sex hormones potentially cause neuroplasticity changes through four neurochemical neurotransmitter systems: serotonin, dopamine, GABA and glutamate. Many brain regions express high density estrogen and progesterone receptors such as the amygdala, hypothalamus, and hippocampus. The hippocampus is of particular importance in the context of mediating structural plasticity in the adult brain, differences in behavior, neurochemical patterns and structure of the hippocampus with a changing hormonal environment have been investigated. There is a significant association between emotion dysregulation and symptoms of depression, anxiety, eating pathology, and substance abuse. Higher levels of emotion regulation are associated with a high level of social competence.

Currently, three main problems have been updated in gerontology and geriatrics: the first “early” is osteoporosis, the second “late” is Alzheimer’s disease, and the third “doubled” (comorbid, combined) is the presence of two diseases: Alzheimer’s disease and osteoporosis. The bone, in addition to mechanical and protective function, serves as a custodian of bone marrow cells and an organ for regulating the homeostasis of calcium ions. In the circadian gerontological period, osteoblasts, osteoclasts and osteocytes are determined by their respective functions of bone formation and bone resorption, and in the clinical-geriatric period of bone aging, bones and bone cells will change. 5P medicine and 5G medical services, new generation sequencing and pharmacogenetics are new modern foundations (paradigms) of personalized medicine — a key basis for understanding the common mechanisms of the pathogenesis of osteoporosis and Alzheimer’s disease. The presence of multiple chronic conditions in gerontology and geriatrics requires the use and clinical application of information and communication infrastructure 5G the next generation, neuromarketing and neurobial, neuroimaging consciousness and brain-machine interfaces, biorobots and biochips. 5P medicine and functioning of integrated neural systems through integration and analysis of dynamic hybrid multimodal neural EEG and fMRI information, combined with neuropsychological testing, will allow the geriatrician to manage (prevent and predict), not only healthy aging of Homo sapiens, but also timely diagnosis of osteoporosis and Alzheimer’s disease, treatment start time, definition. Early age (from 40–45 years) diagnosis, treatment and prevention of osteoporosis using tools (technologies) 5P medicine and 5G medical services, will manage the medical, socio-economic problem of our time. Early age diagnosis, treatment and prevention of osteoporosis using tools (technologies) 5P medicine and 5G medical services, allows you to manage a “silent epidemic” (“hidden epidemic”) of the 21st century with the help of non-medicamentous treatments — three multi-paradigm platforms: healthy lifestyle, physical activity, functional nutrition (balanced, healthy, natural). The key to the success of osteoporosis treatment is in a new 5P medicine based on a deep individualized approach and motivation to prevent diseases.
