Abstract. We collected for rural western China data for 14,658 children less than 36 months of age in 2001 and for 14,112 children in 2005. A generalized estimated equation log-binomial model was used to identify the determinants of childhood diarrhea. In 2005, prevalence of diarrhea was approximately 7.4%, which compared with that for 2001, had decreased by 8.7%. The generalized estimated equation analysis showed that region affected the prevalence of childhood diarrhea significantly. Age was negatively associated with childhood diarrhea. Although childhood upper respiratory infections increased the risk of diarrhea, the risk could be decreased by use of vitamin A in the previous year. In addition, children of Han ethnicity or those living in one-child families had a lower risk of diarrhea in 2001, but underweight children had a higher risk in 2005. These findings may have some implications for formulating policies of childhood diarrhea prevention and control in rural western China.

Abstract. Childhood diarrhea is an important cause of malnutrition, which can be worsened when caretakers limit nutritional support. We queried 390 caregivers and their children in a peri-urban community in Lima, Peru regarding general perceptions of feeding and feeding practices during diarrhea. Overall, 22.1% of caregivers perceived feeding during diarrhea to be harmful. At baseline, 71.9% of caregivers would discontinue normal feeding or give less food. Most would withhold milk, eggs, and meats. Approximately 40% of caregivers would withhold vegetables and fruits. A pilot educational intervention was performed to improve feeding during diarrhea. At follow-up survey 3 months later, none of the caregivers would recommend withholding food. Only 23.2% would recommend discontinuing normal feeding and 1.8% perceived food to be damaging. Misperceptions of the role of feeding during diarrhea pose a significant health risk for children, but a simple educational intervention might have a major impact on these perceptions and practices.

Objective: The present analysis aimed to observe nutritional impacts among children <5 years of age by mother's engagement in paid employment. Design: Between 1996 and 2012, 21 443 children <5 years of age with diarrhoea attended the International Centre for Diarrhoeal Disease Research, Bangladesh (icddr, b), Dhaka Hospital. They were enrolled in the hospital-based Diarrhoeal Disease Surveillance System and their relevant information was extracted from the electronic database. Setting: The icddr,b, Bangladesh. Subjects: The analytic sample was 19 597 children aged <5 years who had a mother aged ≤35 years with or without engagement in paid employment. Results: Eleven per cent of the mothers (n 2051) were currently engaged in paid employment on behalf of the family. Univariate analysis showed that children with mothers engaged in paid employment had a 1·14 times higher risk of being undernourished, a 1·20 times of higher risk of being stunted, a 1·21 times higher risk of being wasted and a 1·31 times higher risk of being underweight (risk ratios) than were children with mothers not likewise engaged. Multivariate analysis showed that such associations remained significant for stunting (1·08; 95 % CI 1·00, 1·16), wasting (1·15; 95 % CI 1·06, 1·25) and underweight (1·09; 95 % CI 1·02, 1·17) after controlling for covariates. Conclusions: Mothers' engagement in income-generating employment was associated with undernutrition in children <5 years of age in urban Bangladesh.
