Severe hemoptysis from dilated systemic aberrant arteries supplying normal lung segments

Abstract: Hemoptysis is an uncommon presentation in children. It can be caused by several systemic and pulmonary disorders. Bleeding from an anomalous arterial supply to normal lung segments with no underlying pulmonary or cardiovascular disorders was widely reported in adults but is extremely rare in the pediatric age group. Here we describe 4 previously normal children and one girl with chronic lung disease, secondary to bronchopumonary dysplasia, with acute and significant hemoptysis of variable severity. Extensive c… Show more

“…reported an 11‐year‐old girl with bleeding from a hypertrophied bronchial artery. Abdulhamid and Forbes reported five patients aged 7–15 years with aberrant systemic arteries from the aorta, right iliac artery, and left/right internal mammary artery. The longest follow‐up time after BAE without complication was reported as 2 years.…”

“…Pulmonary sequestration is characterized by a lack of normal bronchial connections to the segment of the lungs supplied by the anomalous vessel. Anomalous systemic arterial supply to a normal segment of the lung (i.e., without sequestration) is a much rarer entity described in case reports in children [1][2][3][4][5][6] and adults [7][8][9][10][11]. This is previously classified as Pryce type I sequestration [12].…”

“…While most patients are asymptomatic, haemoptysis and exertional dyspnoea are the most common presenting symptoms. If the amount of shunt is sufficiently large to cause cardiac overload, congestive heart failure may develop 2 …”
