Severe Consumptive Coagulopathy in an Extremely-Low-Birth-Weight Infant with Intra-Abdominal Umbilical Vein Varix: A Case Report

Abstract: Recent studies have shown favorable outcomes for intra-abdominal umbilical vein varices (IUVVs) in term neonates who have no other complications. Little is known, however, about the prognosis of IUVVs in preterm neonates. We encountered a case of IUVV in an extremely low-birth-weight infant who developed severe consumptive coagulopathy after birth. The patient's coagulation test normalized as the varix spontaneously obstructed. Although life-threatening hemorrhagic complications were avoided, a cerebellum hemo… Show more

“…Despite increased vulnerability to thrombosis due to UVV during pregnancy, coagulopathy and DIC seem rare. Only four cases have been reported, and all four are preterm neonates born before 34 weeks of gestation ( 9 ). Plasma activities of coagulation factors in preterm neonates are reduced and the fibrinolysis system shows reduced plasminogen activity and increased plasminogen activator inhibitor activity.…”

“…Plasma activities of coagulation factors in preterm neonates are reduced and the fibrinolysis system shows reduced plasminogen activity and increased plasminogen activator inhibitor activity. Furthermore, the presence of UVV could be a predisposition for thrombosis, resulting in the consumption of coagulation factors and the induction of fibrinolysis ( 9 , 10 ). Premature neonates could be more susceptible to thrombosis and coagulopathy.…”

“…The FIUVV was diagnosed at the gestational age of 19w1d with a rapid increase in the size of the varix. In the neonatal period complications includes consumptive coagulopathy and cerebral hemorrhage have been reported recently [13].…”
