Severe acute tumor lysis syndrome in patients with germ-cell tumors

Abstract: Germ-cell tumors are a high-proliferative type of cancer that may evolve to significant bulky disease. Tumor lysis syndrome is rarely reported in this setting. The reports of three patients with germ-cell tumors who developed severe acute tumor lysis syndrome following the start of their anticancer therapy are presented. All patients developed renal dysfunction and multiorgan failure. Patients with extensive germ-cell tumors should be kept on close clinical and laboratory monitoring. Physicians should be aware… Show more

“…8 Adequate hydration and the administration of allopurinol and rasburicase are recommended, but these preventive measures might be inadequate; one case describes acute renal failure occurring despite their use. 9 Currently, there is also no effective prevention for CS.…”

“…One of the most critical and life-threatening adverse conditions of chemotherapy needing immediate intervention is tumour lysis syndrome. It results from the massive and abrupt destruction and lysis of malignant cells and subsequent release of intracellular ions and metabolites into the bloodstream leading to hyperuricemia, hyperphosphatemia, hypocalcemia, and hyperkalemia [57, 58]. This syndrome is accompanied by renal failure and metabolic acidosis increasing the risk of death [57].…”

Although the tumour lysis syndrome is most frequently observed in patients with haematologic malignancies, it may also occur in solid tumours [57, 58].…”

“…These tumors are characterized by high mitotic rates and rapid proliferation, often resulting in bulky tumor burden. Tumor lysis syndrome has been reported in association with germ cell tumors, typically following chemotherapy ( Feres et al, 2008 ). Herein, we present the case of tumor lysis syndrome following low-dose, localized palliative radiation prior to chemotherapy exposure in a patient with a metastatic germ cell tumor.…”
