Seven-year follow-up of combined cataract extraction and viscocanalostomy

Abstract: Phacoviscocanalostomy was safe and effective for the management of eyes with coexisting cataract and medically uncontrolled glaucoma. It provided a stable and sustained reduction in IOP with a minimum requirement for topical medication.

“…Removal of the crystalline lens could potentially improve outflow by further increasing trabecular meshwork tensioning in conjunction with canaloplasty. Other studies investigating non-penetrating glaucoma surgery in combination with phacoemulsification cataract surgery are supportive of this combined beneficial effect [32][33][34][35][36].…”

“…This fi nding may explain the different rates of IOP reduction after VCS with different study protocols, especially in eyes with different preoperative IOP levels. 2,9,18,20 Levene 21 studied post-trabeculectomy eyes with a successfully formed bleb and found that the net IOP reduction resulting from trabeculectomy was proportional to the preoperative IOP and proposed the following formula: IOP reduction = 0.93 × baseline pressure − 9.7 (correlation coeffi cient, 0.90; P = 0.0001; intercept, 9.7 ± 2.2). This formula is quite similar to ours except for the larger intercept of 14.551 in Lectomy-MMC cases in our results.…”

“…11 Endoscopically guided laser goniotomy 12 has also been reported to be safely combined with cataract surgery, with results comparable to those of phacotrabeculectomy. Widely used techniques in glaucoma-cataract surgery are phacoviscocanalostomy, 13 phacoemulsification and deep sclerectomy, 4,14 or phacotrabeculotomy 15,16 because all these methods are safer than phacotrabeculectomy in reducing overfiltration and hypotony-related complications.…”
