Serum Troponin in Neonatal Intensive Care

Abstract: Cardiac troponins have a major role in screening and diagnosis of myocardial ischaemia in adults and children. Their introduction has redefined the diagnosis of myocardial infarction in adults and provided valuable prognostic information. In the paediatric population, troponins show a good correlation with the extent of myocardial damage following cardiac surgery and cardiotoxic medication, and can be used as predictors of subsequent cardiac recovery and mortality. This review discusses the current established… Show more

“…Cardiac troponins (cTn) have a high sensitivity and specificity for myocardial injury. Troponin consists of three subunits: troponin I (cTn-I), troponin T (cTn-T), and troponin C. Troponin T and I are ideal diagnostic markers for myocardial injury since the heart and extracardiac muscle express distinct isoforms which are encoded by different genes (El-Khuffash and Molloy, 2008;Undhad et al, 2012). Owing to the positive correlation between the concentration of troponin and the size of myocardial injury developing after cardiac surgery or cardiotoxic drug treatment, troponin is useful as a prognostic indicator of mortality and morbidity in the paediatric age group.…”

“…Son yıllarda yenidoğan döneminde kardiyak troponinlerin, asfiktik bebeklerdeki miyokard hasarını yansıtabileceği tartışılırken, birçok merkez bu amaçla kardiyak troponin düzeylerini incelemektedir (7,8,22,24). Türker ve ark.…”

“…Several hypotheses have been proposed. It could be that the perinatal period causes minimal myocardial compromise; alternatively, the rise may be a result of perinatal cardiovascular remodeling in the course of adapting to extrauterine life, possibly influenced by respiratory compromise (31,32). No relationship was found between elevated troponin levels and the method of delivery (33,34).…”

“…No relationship was found between elevated troponin levels and the method of delivery (33,34). Reference values are available, but, especially in the case of cTnI, these are related to the type of assay used (31). As discussed earlier, this is an important limitation for routine clinical use.…”
