Serum thyroglobulin determinations in the differential diagnosis of congenital hypothyroidism

Abstract: The value of serum thyroglobulin (Tg) determination in the differential diagnosis of congenital hypothyroidism was assessed by serum Tg measurements in 14 patients with proven congenital hypothyroidism, in 3 subjects with transient perinatal hypothyroidism, in 3 newborns with congenital thyroxine binding globulin (TBG) deficiency and in 34 normal controls. Serum Tg was undetectable in all 6 cases with thyroid agenesis, normal or moderately elevated in the 4 cases with ectopic thyroid, markedly increased in the… Show more

“…Some causes are clearly related to potentially heritable abnormalities because thyroid dysgenesis and others are heritable and may result from a spectrum of genetic defects. The Tg serum level associated with other imaging parameters is important for establishing the etiology of congenital hypothyroidismspecifically, newborns having dyshormonogenesis (19)(20)(21)(22)(23)(24)(25)(26)30). The Tg concentration may also be important for the molecular characterization of congenital hypothyroidism and for counseling in the future.…”

“…Tg evaluation along with imaging studies is a powerful tool for the etiologic diagnosis of congenital hypothyroidism and for the elucidation of the molecular disturbance involved (19)(20)(21)(22)(23)(24)(25)(26). Scant studies reporting Tg reference ranges measured by current methods are available for the first months of life.…”

“…complete absence of thyroid tissue, is very rare in the Swedish CH population. Various frequencies of aplasia have been reported by other authors (8)(9)16). Determination of the Tg concentration can be used as a substitute for scintigraphy in CH patients for detecting total aplasia of the thyroid gland.…”

“…It has been shown that many patients with permanent CH have measurable Tg levels in serum [18][19][20][21][22][23]. These Tg values are believed to be due to secretion from residual thyroid tissue.…”
