Serum levels of mannose‐binding lectin and the risk of fever in neutropenia pediatric cancer patients

Abstract: Very low MBL levels are associated with more frequent FN episodes, mainly due to severe bacterial infections. The surprising finding that children with normal MBL levels had more frequent FN episodes than those with low MBL levels needs testing in prospective studies.

“…25 Levels p400 ng/ml correlated highly with YO/YO, XA/YB or XA/YC genotypes in 500 healthy children (own unpublished observations). Our data are consistent with earlier reports showing an association between low MBL levels and increased infection rates in haematological patients with neutropenia 6,7 and of a protective role of WT MBL genotype during Auto-SCT. 9 In addition, we observed an increased rate of sepsis, not only in patients with low MBL levels but also in patients with very high levels.…”

“…Earlier studies have suggested an influence of MBL status on infection in neutropenic patients. 6,7 We analysed 131 stem cell recipients to clarify the role of donors' and recipients' MBL genotypes and MBL serum levels on infectious and transplant-related complications.…”

“…In patients with a hematological malignancy receiving chemotherapy, increased susceptibility to infection and prolonged duration of febrile neutropenic episodes were associated with MBL deficiency. [4][5][6][7][8] However, these observations are not consistent in all studied cohorts. [9][10][11] Few studies have included hematopoietic SCT (HSCT) recipients 7,8,10 or have focused on children.…”

“…[9][10][11] Few studies have included hematopoietic SCT (HSCT) recipients 7,8,10 or have focused on children. 4,6,9 MBL deficiency can be corrected by liver transplantation, whereas MBL deficiency can be induced by transplantation of MBL-sufficient recipients with an MBL-deficient donor liver. 12 Whether the donor MBL status has an impact on the MBL status or infection susceptibility of the recipient post-HSCT is unclear.…”
