Background: Henoch-Schönlein purpura nephritis (HSPN) is the principal cause of morbidity and mortality in children with Henoch-Schönlein purpura (HSP). However, the criteria for risk assessment currently used is not satisfactory. The urine proteome may provide important clues to indicate the development of HSPN. Methods: Here, we detected and compared the urine proteome of patients with HSPN and healthy controls by liquid chromatography-tandem mass spectrometry (LC-MS/MS) in the data-independent acquisition (DIA) mode. The differentially expressed proteins were analysed by gene ontology (GO) analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) analysis. For validation, enzyme-linked immunosorbent assay (ELISA) was used to analyse the selected proteins. Results: A total of 125 proteins (29 upregulated and 96 downregulated) were found to be differentially expressed in children with HSPN compared with the controls. Forty-one proteins were predicted to have direct interactions. The enriched pathways mainly included focal adhesion, cell adhesion molecules, the PI3K-Akt signalling pathway, ECMreceptor interactions and so on. Cell adhesion related to the pathogenesis of HSPN was the main biological process identified in this study. The decrease in two proteins (integrin beta-1 and tenascin) was validated by ELISA. Conclusions: Our study provides new insights into the assessment of HSPN progression in children, as well as new potential biomarkers. The data confirm the value of the urinary proteome in capturing the emergence of HSPN.

ΣΚΟΠΟΣ: Η μελέτη είχε στόχο να ερευνήσει την σπειραματική έκφραση των μεταλλοπρωτεϊνασών (ΜΜΡs) MMP-1, MMP-2, MMP-3, MMP-9 και του αναστολέα τους ΤΙΜΡ-1 σε περιπτώσεις σπειραματονεφρίτιδας ασθενών με Συστηματικό Ερυθηματώδη Λύκο (ΣΕΛ) και να μελετήσει τη συσχέτισή τους με συγκεκριμένους δείκτες ενεργότητας καθώς και με την νεφρική λειτουργία, τη στιγμή της διενέργειας της νεφρικής βιοψίας. ΜΕΘΟΔΟΣ: Η αξιολόγηση της έκφρασης των ΜΜΡ έγινε με ανοσοϊστοχημεία. Για τις χρώσεις χρησιμοποιήθηκαν μονοκλωνικά αντισώματα έναντι των MMP-1, MMP-2, MMP-3, MMP-9 και TIMP-1 σε 58 περιπτώσεις βιοψίας νεφρού ασθενών με ΣΕΛ νεφρίτιδα. Η αξιολόγηση των βιοψιών έγινε με βάση το σύστημα κατάταξης 2004 ISN/RPS. Ως μάρτυρες χρησιμοποιήθηκαν 8 δείγματα φυσιολογικού νεφρικού παρεγχύματος από περιπτώσεις νεφρεκτομής λόγω τραυματισμού ή νεοπλασίας. Για τη συσχέτιση με τη νεφρική λειτουργία χρησιμοποιήθηκαν κλινικά και εργαστηριακά δεδομένα από 36 ασθενείς. ΑΠΟΤΕΛΕΣΜΑΤΑ: Στη μελέτη φάνηκε ισχυρή συσχέτιση της σπειραματικής έκφρασης των ΜΜΡ-1, ΜΜΡ-2 και ΜΜΡ-3 με τους περισσότερους επιμέρους δείκτες ενεργότητας, με υψηλότερο συνολικό δείκτη ενεργότητας και με χειρότερη νεφρική λειτουργία κατά τη στιγμή της διενέργειας της νεφρικής βιοψίας (p<0.001). Ιδιαίτερη συσχέτιση διαπιστώθηκε μεταξύ της έκφρασης της ΜΜΡ-1 και της λευκοκυτταρικής διήθησης (OR:16,5 95%CI 4,3-62,5 p<0,001) καθώς και της έκφρασης της ΜΜΡ-3 με την ενδοτριχοειδική κυτταροβρίθεια (OR:18,6 95%CI 4,8-72,8 p<0,001). Επίσης, ισχυρή συσχέτιση διαπιστώθηκε μεταξύ της έκφρασης της ΜΜΡ-2 και της παρουσίας ινιδοειδούς νέκρωσης και κυτταρικών μηνοειδών σχηματισμών (OR:17,1 95%CI 4,3-67,7 p<0,001), η οποία παρουσίασε ιδιαίτερο ενδιαφέρον. ΣΥΜΠΕΡΑΣΜΑ: Στη σπειραματονεφρίτιδα του ΣΕΛ παρατηρείται αυξημένη έκφραση των μεταλλοπρωτεϊνασών, η οποία παρουσιάζει σημαντική συσχέτιση με τις επιμέρους παραμέτρους ενεργότητας καθώς και το συνολικό δείκτη ενεργότητας. Το φαινόμενο αυτό σχετίζεται με εντονότερη φλεγμονώδη αντίδραση, σημαντικότερη παρεγχυματική βλάβη και με χειρότερη νεφρική λειτουργία κατά τη στιγμή της διενέργειας της βιοψίας.

End-stage renal disease (ESRD) is a serious disease that requires hemodialysis as the main regimen for supporting patients. The pathophysiology of the disease is still an interesting field of study. In the present study, matrix metalloproteinase-3 (MMP3), a tissue inhibitor of metalloproteinase-1 (TIMP1), and vitamin D (VitD) were studied in ESRD. MMP3, TIMP1, VitD, and other related biomarkers were measured in 53 ESRD patients and compared with 33 healthy controls. The results showed a significant increase in serum MMP3 and TIMP1 and a significant decrease in serum VitD in ESRD patients compared with the control group. The multivariate GLM results revealed no significant effects of the covariates (sex, age, smoking and BMI) on the levels of MMP3, TIMP1 and VitD. ESRD was found to be the major factor affecting the measured parameters, with a huge effect size (Partial η2) of 0.824. According to our data, eGFR, Creatinine, Urea, phosphate, uric acid and VitD (as estimated glomerular filtration rate) are the top 6 factors highly affected by the presence of the disease. MMP3 and TIMP1 are important parameters in ESRD and may act as drug targets. As the patients also suffered from hypovitaminosis D and hypocalcemia, these factors also need intervention.
