Objective: To examine the relationship between fasting plasma leptin and 24-hour energy expenditure (EE), substrate oxidation, and spontaneous physical activity (SPA) in obese subjects before and after a major weight reduction compared with normal weight controls. To test fasting plasma leptin, substrate oxidations, and SPA as predictive markers of success during a standardized weight loss intervention. Research Methods and Procedures: Twenty-one nondiabetic obese (body mass index: 33.9 to 43.8 kg/m 2 ) and 13 lean (body mass index: 20.4 to 24.7 kg/m 2 ) men matched for age and height were included in the study. All obese subjects were reexamined after a mean weight loss of 19.2 kg (95% confidence interval: 15.1-23.4 kg) achieved by 16 weeks of dietary intervention followed by 8 weeks of weight stability. Twenty-four-hour EE and substrate oxidations were measured by whole-body indirect calorimetry. SPA was assessed by microwave radar. Results: In lean subjects, leptin adjusted for fat mass (FM) was correlated to 24-hour EE before (r ϭ Ϫ0.56, p Ͻ 0.05) but not after adjustment for fat free mass. In obese subjects, leptin correlated inversely with 24-hour and resting nonprotein respiratory quotient (r ϭ Ϫ0.47, p Ͻ 0.05 and r ϭ Ϫ0.50, p Ͻ 0.05) both before and after adjustments for energy balance. Baseline plasma leptin concentration, adjusted for differences in FM, was inversely related to the size of weight loss after 8 weeks (r ϭ Ϫ0.41, p ϭ 0.07), 16 weeks (r ϭ Ϫ0.51, p Ͻ 0.05), and 24 weeks (r ϭ Ϫ0.50, p Ͻ 0.05). Discussion: The present study suggests that leptin may have a stimulating effect on fat oxidation in obese subjects. A low leptin level for a given FM was associated with a greater weight loss, suggesting that obese subjects with greater leptin sensitivities are more successful in reducing weight.

White adipose tissue development is regulated by many factors, including the energy content of food and the genetic background. Nevertheless, little is known about possible differential effects of high-fat palatable diets when fed for short or long-time periods. Thus, the expression of certain genes involved with lipid metabolism (peroxisome proliferator-activated receptor gamma, PPAR␥2; retinoic receptors; fatty acid binding protein, aP2 and uncoupling proteins, UCP) may be affected by those dietary manipulations (highenergy-yielding diet and time duration of feeding). High-fat feeding for 8 days decreased mRNA UCP3 levels compared to control fed animals, while feeding for 30 days increased them over controls. Similar findings occurred for PPAR␥2 and aP2. Furthermore, statistically significant associations were found among PPAR␥2, aP2 and UCP3 mRNA levels. These data suggest a physiological time-dependent response seeking to prevent excessive fat deposition when animals are fed for short-term with a high amount of dietary fat, which was followed by an adaptive period to the highenergy content of diet throughout a coregulation among certain lipid metabolism related genes: PPAR␥2, aP2, UCP3.

Arq Bras Endocrinol Metab vol 47 nº 4 Agosto 2003 398 RESUMOO peso corporal é regulado por uma interação complexa entre hormônios e neuropeptídeos, sob o controle principal de núcleos hipotalâmicos. Mutações nos genes de hormônios e neuropeptídeos, de seus receptores ou de elementos regulatórios, têm sido descritas na espécie humana, mas são tidas como raras, não explicando as formas mais comuns de obesidade. No entanto, o estudo destas mutações tem propiciado um grande avanço nos conhecimentos sobre a base genéti-ca e a fisiopatologia da obesidade, possibilitando o estudo e abrindo perspectivas para o desenvolvimento de novas modalidades terapêuti-cas. Recentemente, demonstrou-se que mutações no receptor 4 da melanocortina podiam ser encontradas em até 5% dos casos de obesidade severa, representando até o presente momento a forma mais prevalente de obesidade monogênica na espécie humana. Nesta revisão, são discutidas as diversas mutações descritas nos seres humanos de elementos da rede neuroendócrina de controle do peso corporal, bem como as implicações dos mesmos na gênese da obesidade. A complex hypothalamic network of hormones and neuropeptides regulates body weight. Mutations in these hormones/peptides, their receptors or regulatory elements, have been described in humans, but they are rare and could not explain the commonest forms of obesity. Nevertheless, the study of these mutations has favored a great progress in the knowledge of genetic basis and physiopathology of obesity, opening new perspectives on the therapeutic approach of this prevalent disease. Recently, mutations in the melanocortin 4 receptor have been found in up to 5% of severe obese subjects, being thus far the most prevalent monogenic form of obesity in humans.
