Mineral and bone metabolism are regulated differently in utero compared with the adult. The fetal kidneys, intestines, and skeleton are not dominant sources of mineral supply for the fetus. Instead, the placenta meets the fetal need for mineral by actively transporting calcium, phosphorus, and magnesium from the maternal circulation. These minerals are maintained in the fetal circulation at higher concentrations than in the mother and normal adult, and such high levels appear necessary for the developing skeleton to accrete a normal amount of mineral by term. Parathyroid hormone (PTH) and calcitriol circulate at low concentrations in the fetal circulation. Fetal bone development and the regulation of serum minerals are critically dependent on PTH and PTH-related protein, but not vitamin D/calcitriol, fibroblast growth factor-23, calcitonin, or the sex steroids. After birth, the serum calcium falls and phosphorus rises before gradually reaching adult values over the subsequent 24 -48 h. The intestines are the main source of mineral for the neonate, while the kidneys reabsorb mineral, and bone turnover contributes mineral to the circulation. This switch in the regulation of mineral homeostasis is triggered by loss of the placenta and a postnatal fall in serum calcium, and is followed in sequence by a rise in PTH and then an increase in calcitriol. Intestinal calcium absorption is initially a passive process facilitated by lactose, but later becomes active and calcitriol-dependent. However, calcitriol's role can be bypassed by increasing the calcium content of the diet, or by parenteral administration of calcium.

WHAT'S KNOWN ON THIS SUBJECT: A serum 25-hydroxyvitamin D (25(OH)D) concentration of 20 ng/mL meets the requirements of at least 97.5% of the population older than 1 year. A recommended dietary intake to achieve this serum 25(OH)D concentration has not been established during infancy. WHAT THIS STUDY ADDS: METHODS:A randomized, double-blind, placebo-controlled trial in New Zealand. Pregnant mothers, from 27 weeks' gestation to birth, and then their infants, from birth to age 6 months, were randomly assigned to 1 of 3 mother/infant groups: placebo/placebo, vitamin D 3 1000/400 IU, or vitamin D 3 2000/800 IU. Serum 25(OH)D and calcium concentrations were measured at enrollment, 36 weeks' gestation, in cord blood, and in infants at 2, 4, and 6 months of age.RESULTS: Two-hundred-and-sixty pregnant women were randomized. At enrollment, the proportions with serum 25(OH)D $20 ng/mL for placebo, lower-dose, and higher-dose groups were 54%, 64%, and 55%, respectively. The proportion with 25(OH)D $20 ng/mL was larger in both intervention groups at 36 weeks' gestation (50%, 91%, 89%, P , .001). In comparison with placebo, the proportion of infants with 25(OH)D $20 ng/mL was larger in both intervention groups to age 4 months: cord blood (22%, 72%, 71%, P , .001), 2 months (50%, 82%, 92%, P , .001), and 4 months (66%, 87%, 87%, P = .004), but only in the higher-dose group at age 6 months (74%, 82%, 89%, P = .07; higher dose versus placebo P = .03, lower dose versus placebo P = .21).

1Московский государственный медико-стоматологический университет, 2 ГУ НИИ питания РАМН В последнее десятилетие возрос интерес к проблеме остеопороза в педиатрии [6], поскольку именно в детстве закладывается та прочность скелета, а в пубертате эффективно накапливается такая пиковая костная масса, от которых впоследствии во многом зависит предрасположенность к переломам на протяжении всей будущей жизни. В настоящем обзоре суммированы современные представления о нормальной физиологии обмена кальция и витамина D, а также о специфических особенностях минерального обмена костной ткани в возрастном аспекте (беременная→ плод→новорождённый →ребёнок→подросток). Регуляция обмена кальция и фосфора,новые данные о роли витамина D Среди факторов, оказывающих решающее влияние на процессы остеогенеза, рост и форми-рование здорового скелета, достижение им опти-мальной, генетически детерминированной пико-вой массы, главенствующая роль принадлежит правильному питанию [8,9] и, прежде всего, надежному обеспечению растущего организма всеми минеральными веществами и незаменимыми витаминами.1. При обсуждении конкретной роли различных микро-нутриентов, их взаимодействия и взаимосвязи в процессах остеогенеза первостепенную роль отводят кальцию (Са).При появлении на свет в теле новорождённого общее количество Са составляет всего около 25 г [13,22, 32,33], но по мере созревания скелета эта величина возрастает до 1,3-1,5 кг. В период бурного роста, когда подростки наращивают более трети общей «взрослой» массы костей, предъявляют-ся наиболее жёсткие требования к полному обеспечению кальцием растущего организма [15, 16, 34].Концентрация общего (свободного и связанного) Са, а также его свободной фракции -ионизированного Са в плазме крови здорового ребёнка находится в достаточно узком диапазоне (2,2-2,6 ммоль/л и 1,0-1,2 ммоль/л со-ответственно).Жёсткое поддержание концентрации Са в указанных пре-делах имеет исключительно важное значение [8]. Снижение уровня ионизированного Са ниже 0,6-0,7 ммоль/л ведёт к нарушениям минерализации костей, снижению и утрате мы-шечного тонуса, повышенной возбудимости двигательных нейронов и тетаническим судорогам, в то время как гипер-кальциемия оказывает повреждающее действие на многие ферментные системы и клеточные функции, вызывает нару-шения сердечной деятельности, кальциноз почек, сердца, аор-ты, коронарных сосудов, базальных ганглиев головного мозга и других тканей с необратимым расстройством их функций.Процесс роста и формирования скелета сводится не к простому увеличению массы костной ткани и степени ее минерализации [9]. Скелет, как и любой другой орган или ткань, находится в состоянии постоянного самообновления, ремоделирования, являющегося результатом двух одновре-менно идущих процессов: резорбции, рассасывания уже су-ществующей кости, осуществляемой остеокластами, и ее об-разования, моделирования остеобластами.
