To determine if the cyclic changes of female sex hormones during the menstrual cycle are related to changes in bone formation and resorption, we measured serum bone-specific alkaline phosphatase (BAP) and osteocalcin (OC) and bone resorption markers, serum and urine deoxypyridinoline (Dpyr), three times per week during one menstrual cycle in 20 healthy premenopausal women. Summary.-Serum concentrations of oestradiol-17/ were measured by radioimmunoassay during 40 menstrual cycles from 38 normal premenopausal women. Repeated samples were studied from 25 normal postmenopausal women. The method was highly specific and extremely sensitive. In premenopausal women the pattern of serum oestradiol-17/ was remarkably constant but the concentrations varied with age. Women in the fourth decade of life had significantly higher concentrations of oestradiol-17/ than either younger or older women. In postmenopausal women the concentration of oestradiol-17f was consistently very low.IT seems clear from clinical (Schinzinger, 1889;Beatson, 1896), experimental (Loeb, 1919;Huggins, Briziarelli and Sutton, 1959) and biochemical evidence (Brown, 1958;Marmorston et al., 1965;Persson and Risholm, 1964;Nissen-Meyer and Sanner, 1963) necessary to examine daily samples taken throughout the menstrual cycle. In this study a group of normal premenopausal women was studied daily in an attempt to define the normal range and relate it to the age of the subjects. A further group of postmenopausal and menopausal women was studied repeatedly in a similar way. The object of the study was to provide a normal baseline with which to compare patients with benign and malignant breast disease. MATERIALS AND METHODSSamples of blood were taken daily or as often as possible during at least one menstrual cycle from 38 normal premenopausal women. Multiple samples were also obtained from 25 normal postmenopausal and 3 menopausal women (within one year of cessation of periods). None had a history of breast disease or gynaecological abnormality and none was taking any form of hormonal preparation. The ages of the 38 premenopausal subjects ranged from 21 to 53 years (mean 34 8). Of these, 14 were aged 20-29 years, 13 were 30-39 years, 10 were 40-49 years and one was 53 years of age. The ages of the normal postmenopausal subjects ranged from 47 to 64 years (mean 56).

RESUMOEste artigo irá rever os diversos aspectos da falência ovariana prematura (FOP), definida como o desenvolvimento de hipogonadismo em mulheres antes dos 40 anos, desde a etiopatogenia, com discussões sobre as causas autoimunes, iatrogênicas, ou anormalidades do cromossomo X assim como manifestações clínicas, diagnóstico e tratamento. A maioria das mulheres com essa condição não possui uma história menstrual, específica do desenvolvimento da FOP, mas a infertilidade associada ao diagnóstico é o aspecto mais problemático da doença. INTRODUÇÃO M uitas revisões recentes têm discutido o nome apropriado para essa condição, embora muitos pacientes e médicos prefiram o termo menopausa prematura, um termo simples e compreensível que pode ser explicado aos amigos e familiares, mas implica certa sensação de permanência, no entanto, a ovulação e a gravidez podem ocorrer em um momento posterior, por vezes anos após o diagnóstico (1-3).A falência ovariana precoce (FOP) é definida como falência gonadal antes dos 40 anos de idade. A incidência é de aproximadamente 1 em 250 mulheres com idade de 35 anos e de 1 em 100 com idade de 40 anos. É idiopática em 74% a 90% das pacientes. Algumas pacientes com FOP podem produzir estrogênio, ovular, e, em 5% a 10% dos casos, conceber e ter uma gravidez e parto sem alterações, assim como ter a função ovariana intermitente (sem amenorreia associada) e menstruação espontânea (embora anormalidades da função menstrual estejam presentes) (3-9).Neste artigo, o termo a ser utilizado será falência ovariana precoce (FOP), o qual é adequado e descreve seu diagnóstico. ETIOLOGIA E PATOGENIAEntender a causa da FOP exige uma compreensão do processo de foliculogênese em que muitas das molé-culas que regulam a série de eventos na maturação folicular estão envolvidas.
