Serum Cystatin C Concentration Compared with Other Markers of Glomerular Filtration Rate in the Old Old

Abstract: Serum cystatin C, serum creatinine, the Cockcroft-Gault formula, the MDRD formula, and urinary creatinine clearance are comparable markers of renal function in the overall older population. The Cockcroft-Gault formula underestimates renal function in older people with GFR greater than 60 mL/min. In our study, serum cystatin C was not superior to serum creatinine in the detection of renal impairment.

“…Van Den Noortgate et al evaluated and compared the concentration of serum Cys-C with markers of GFR in 48 elderly patients, including 25 patients aged 85 years or older. The results of these authors revealed that serum Cys-C, serum Cre, urinary CrCl, the Cockcroft-Gault formula and the MDRD formula were comparable markers of renal function in the overall older population (22). In a recent review, Van Pottelbergh et al indicated that evidence regarding the use of serum levels of Cys-C in elderly patients is limited (6).…”

“…A simple comparison of correlations is not, however, statistically sufficient to confirm that CysC is superior to creatinine. CysC could be a more sensitive marker than creatinine to investigate for moderate reductions in GFR in the elderly (69-92 years old) (185,186), although the results reported are inconclusive (98,99).…”

“…6 As a consequence, clinical guidelines by the National Kidney Foundation (NKF) 7 and others 8 call for screening for CKD using either the Cockcroft-Gault (CG) 9 or Modified Diet in Renal Disease (MDRD) 10 equations, which can be easily calculated, instead of serum creatinine. Of the two, the Modified Diet in Renal Disease (MDRD) equation appears to be more accurate for older patients, [11][12][13][14][15] and is recommended by the NKF.…”
