Seroprevalence of cytotoxin‐associated gene A positive Helicobacter pylori strains in Changle, an area with very high prevalence of gastric cancer in South China

Abstract: Subjects in Changle had a high prevalence of H. pylori infection and a high prevalence of the CagA-positive strains. The contrast in the prevalence of CagA-positive strains, in asymptomatic subjects in two areas with differing gastric cancer mortality, supports the pathogenic role of CagA-positive strains in gastric carcinogenesis.

“…This issue is of particular importance in Asian countries, which have a high prevalence of gastric cancer 16 ,. 17 A lower dosage of proton pump inhibitor with less suppression of intragastric pH, but with a satisfactory reduction in oesophageal acid exposure, may theoretically be ideal for the treatment of acid‐related disorders in a population with a high risk of gastric cancer development. Furthermore, there is a potential cost saving in terms of drug costs with low‐dose proton pump inhibitors.…”

“…Studies done in the West have concluded that infection with cagA ‐positive H. pylori strains is associated with an increased risk of development of atrophic gastritis and intestinal metaplasia [12,13], and hence malignant disease. However, in many parts of Asia, the majority of H. pylori are cagA ‐positive [24–27]. Several studies done in Asian populations have not shown any significant association between cagA and the development of gastric adenocarcinoma [14,15,27,28] because of the background of high prevalence of cagA ‐positive strains in the community.…”

“…The prevalence of cagA in Asia is high, and currently identified cagA genotypes in the Asia-Pacific region have not been associated with gastric cancer. [70][71][72][73][74][75][76] As such, there is no role for it as a marker for gastric cancer risk in an individual.…”

“…Gastric cancer of the cardia was not associated with H. pylori infection or cagA ‐positive strains of H. pylori . However, the prevalence of cagA in Asia is high, and currently identified cagA genotypes in the Asia–Pacific region are not associated with gastric cancer 69–76 …”
