Background: Hepatitis B virus (HBV) can cause chronic hepatitis, cirrhosis, and hepatocellular carcinoma. Coinfection with hepatitis C virus (HCV)/HBV leads to a higher risk of liver damage. Development of HBV awareness campaigns could reduce the prevalence of this disease and limit the consequences. Objectives: The aim of this study was to sensitize, detect, and identify HBV markers and propose vaccination for people not infected with HBV. Methods: With the support of "SOS Hepatitis Burkina" association, an HBV screening program, available for all volunteers, was implemented in several localities of Ouagadougou and the surroundings. A rapid HBsAg detection test was performed on 2207 subjects, who voluntarily answered a series of questions. A rapid detection test of HCV antibodies was performed in HBsAg-positive cases. HBV markers were also determined in HBsAg-positive cases.Results: In a sample of 2207 individuals, aged 1 -85 years (mean age, 31.4 ± 15.7 years), the prevalence of HBsAg was 9.8% (217/2207) after screening. Before screening, 6.8% (150/2207) of the participants claimed that they were vaccinated against HBV. Also, multivariate analysis revealed that HBV infection was significantly associated with age (21 -30 years) and vaccination status (P = 0.03; OR, 1.67; 95% CI, 1.04 to 2.69 and P = 0.003; OR, 5.69; 95% CI, 1.80 to 18.00, respectively). Among 217 HBsAg-positive cases, the prevalence of HBV markers was reported as follows: AbHBs (0.9%), HBeAg (6.0%), AbHBe (87.6%), and AbHBc (100.0%). Based on the findings, HCV was detected in 0.9% (2/217) of HBsAg-positive subjects. In this study, 628 out of 1990 HBsAg-negative subjects were vaccinated for three doses, resulting in a vaccination rate of 31.6%. Conclusions: The present study reported an HBsAg prevalence of 9.8% in the study population. This intervention could contribute to major vaccination coverage against HBV after screening. Therefore, raising awareness and development of screening campaigns on HBV can increase the vaccination coverage of the population. The findings also showed that absence of vaccination against HBV constitutes a high-risk factor for HBV infection.

Africa has the highest prevalence of transfusion transmitted infections. The World Health Organization recommends universal and quality-controlled screening of blood donations for the major transfusion-transmissible infections (TTIs): human immunodeficiency virus (HIV), hepatitis B virus (HBV), hepatitis C virus (HCV) and syphilis. Therefore a retrospective study was conducted to assess the effect of strategies in our blood bank to improve blood safety on the seroprevalence of HIV, HCV, HBV and syphilis infections among the donors over a seven year period. Existing data in the blood bank was used to determine number of the blood donors who were infected with HIV, HCV, HBV and syphilis. The test methods used to screen the donors were identified. The trend of prevalence of the transfusion transmitted infections among the blood donors from 2009 to 2015 was also determine. A total of 41,445 blood donors were screened. Voluntary blood donors constituted 11.1% of the donor population. The overall seroprevalence rate for the TTI was 12.3%. The prevalence was highest for HBV (8.5%) followed by HIV (1.8%), HCV (1.4%) and least for syphilis (0.5%) respectively. The infections showed significant inter-year variation (p<.001). A decreasing trend was observed for HBV among the blood donors while increase in prevalence of HIV, HCV and syphilis was observed from 2012 to 2014 and decreased in 2015. The prevalence of syphilis has risen from 0% in 2009 to 0.9% in 2015. The seroprevalence for TTI is high but is less compared to report from a previous study in same blood bank. The increasing infection rate for syphilis and sporadic surges in rates for HIV, HCV may suggest that the selection criteria is not effectively eliminating blood donors with risky lifestyle. There is need to educate the blood donors on avoiding risky lifestyle while also intensifying voluntary blood donor motivation strategy and increase community surveillance of the infections.

Background: Transfusion transmissible infections, such as HIV, HBV, HCV and syphilis are on the rise and pose a threat to blood safety. Objective: To determine prevalence and demographic profiles of TTI's among first time blood donors in Abeokuta, Nigeria. Methods: The study was conducted between February to November 2013; 130 first time blood donors were tested for the presence of HIV, HBsAg, HCV antibodies and Treponema palidium antibodies using EIA based rapid immunochromatographic kits. Data analysis was done using SPSS with a level of significance of p<0.05. Results: Prevalence rates to HIV, HBsAg, HCV antibody, were 6.2% (n=8), 10% (n=13) and 1.5% (n=2), there was 0% prevalence to Treponema palidium antibodies. Group specific prevalence rates revealed that educational status was associated with HBsAg positivity (p = 0.028), donors with a history of previous blood transfusion was also statistically associated with HIV sero-reactivity (p = 0.013). Conclusions: High levels of HBsAg and HIV were observed, there is need to revise the donor testing algorithm in Nigeria in line with the prevalence of TTI's. We also advocate that a National surveillance system for TTI's be established through our National blood transfusion service (NBTS) program, a second serological test is also suggested to reduce the risk of occult HBV infection in Nigeria.
