SummaryThe prevalence of antibodies againstEntamoeba histolyticawas studied in the Mexican population using an immunoenzyme assay in solid phase (ELISA) and semiautomatic equipment. The antigen was a mixture of membrane proteins obtained by Triton X-100 extraction from an axenic culture ofEntamoeba histolyticaHM1-IMSS. The method was standardized by comparing serum samples from amoebic liver abscess patients with healthy volunteers. From the 60538 samples suppliedbythe National Seroepidemiology Survey, antibodies were found in 4·49 % (4·32–;4·65% at 95 % confidence limit). More significant titres occurred in the central region of the country. The ratio female to male was 1·25:1. The population living in metropolitan areas had probably been infected at a younger age than those living in the country.Important differences were found in the seroprevalence obtained by ELISA compared with a study which used indirect haemagglutination (IHA) in the same sample frame.

609artículos de revisión L a amebiasis se considera como una causa importante de morbilidad y mortalidad en el mundo y constituye dentro de las enfermedades de origen parasitario, especialmente en el mundo en desarrollo, la tercera causa de muerte después de la malaria y la esquistosomiosis 1,2 . El reconocimiento de Entamoeba dispar y Entamoeba moshkovskii en humanos ha complicado más el diagnóstico de E. histolytica. Estas tres especies son morfológicamente indistinguibles y las dos primeras se consideran comensales. Sin embargo, estudios recientes sugieren que puedan desempeñar un papel patógeno. En Bangladesh e India, E. dispar y E. moshkovskii se observaron asociadas con síntomas gastrointestinales 3 y se ha demostrado que E. dispar produce lesiones intestinales y hepáticas en conejillos de Indias 4 . La distribución mundial y la magnitud de la infección de estas tres especies no son conocidas, ya que la mayoría de los diagnósticos, especialmente en el tercer mundo, se siguen basando en la observación microscópica lo que no permite diferenciarlas. Los datos actuales sugieren que E. dispar es 10 veces más común que E. histolytica en individuos asintomáticos pero las prevalencias locales de ambas especies varían significativamente 1 . No todas las infecciones con E. histolytica conducen a la enfermedad y sólo una de cada 10 infecciones progresan hacia la enfermedad 3 . El objetivo de esta revisión es describir y discutir los avances recientes en la amebiasis con énfasis en los aspectos clínicos, de diagnóstico y manejo de la infección. Abstract. To correlate a particular state of immunity with Entamoeba spp., we used colorimetric PCR to differentiate E. histolytica from E. dispar in individuals with amoebiasis and to associate its presence with the clinical profile, including humoral and cellular immune responses to E. histolytica. Our results showed high levels of antibody in acute amoebiasis and elevation of IL-4 production, a cytokine related to Th2 profile, associated with E. histolytica. In chronic amoebiasis, even with anti-E. histolytica seropositivity, intestinal symptoms were associated with E. dispar in all the cases, without differences in level of antibodies, BTI, CD4+/CD8+ ratio, INF-␥, and IL-4. Among asymptomatic carriers, E. dispar was more frequently found; however, identification of E. histolytica in two asymptomatic carriers associated with high levels of INF-␥, a cytokine related to Th1 profile, demonstrate the importance of making specific diagnosis of Entamoeba spp., to establish the clinical and epidemiological behavior in both intestinal and extra-intestinal amoebiasis.
