Semiautomatic Intraosseous Devices in Pediatric Prehospital Care

Abstract: For the pediatric cohort, use of a semiautomatic IO access device in place of a manual device offered no statistically significant difference in first-attempt success (3.3%) or in success per attempt (13.0%). However, the rate at which IO access was used by emergency medical services providers more than tripled with use of the semiautomatic device.

“…Reades and colleagues demonstrated higher success rates with IO line placement compared with IV line placement, 22 but other studies have shown similar success rates for IO placement compared with the IV success rates published herein. 24,25 Because of the lack of an association between prior IV placement success and future success, these data do not answer how best to improve IV success rates for paramedics. Our data suggest that requiring paramedics to perform a certain number of IV placements per year is unlikely to impact future success for full-time paramedics.…”

“…In a pre-hospital study, Myers et al compared Manual IO and a semiautomatic device in children and found success rates of 67.6% in 31 patients using Manual IO and 80.6% in 60 with semiautomatic device. 9 In a second paediatric pre-hospital study, Harthold et al found a success rate of 100% using the Manual IO in12 patients. 10 Other pre-hospital Manual IO studies have shown success rates for children ranging from 78 to 94%.…”
