Segmental Infarct of the Testis Due to Hypersensitivity Angiitis

Abstract: We present a case of hypersensitivity angiitis causing a segmental infarction of the testis. The lesion presented as a discrete, palpable mass that led to orchiectomy. Hypersensitivity angiitis has not been reported previously to affect the testes or to produce a segmental infarct in any solid organ. A discussion is given of the vasculitides, especially periarteritis nodosa, that may involve the testes.

“…It has also been reported secondary to surgery for inguinal hernia repair (31,32), vasectomy (33), or varicocelectomy (34). Other less common predisposing factors for segmental infarction include polycythemia (29), intimal fibroplasia of the spermatic artery (35), sickle cell disease (36) or sickle cell trait (37), hypersensitivity angiitis (38), and trauma. A bell clapper deformity leading to repeated torsion-detorsion episodes is also a predisposing factor for segmental testicular infarct (39).…”

“…Clinically, segmental testicular infarction presents with acute scrotal pain and is indistinguishable from other causes of scrotal pain. A number of factors for segmental testicular infarction have been proposed: epididymo-orchitis, trauma, sickle cell disease, polycythaemia, hypersensitivity angiitis, intimal-fibroplasia of the testicular artery and previous surgery [5][6][7][8][9]. In the majority the cause is attributed to infection, but is termed 'idiopathic' if a cause is not found [7,10].…”
