With the growth of computing and communication technologies, the information processing paradigm of the healthcare environment is evolving. The patient information is stored electronically, making it convenient to store and retrieve patient information remotely when needed. However, evolving the healthcare systems into smart healthcare environments comes with challenges and additional pressures. Internet of Things (IoT) connects things, such as computing devices, through wired or wireless mediums to form a network. There are numerous security vulnerabilities and risks in the existing IoT-based systems due to the lack of intrinsic security technologies. For example, patient medical data, data privacy, data sharing, and convenience are considered imperative for collecting and storing electronic health records (EHR). However, the traditional IoT-based EHR systems cannot deal with these paradigms because of inconsistent security policies and data access structures. Blockchain (BC) technology is a decentralized and distributed ledger that comes in handy in storing patient data and encountering data integrity and confidentiality challenges. Therefore, it is a viable solution for addressing existing IoT data security and privacy challenges. BC paves a tremendous path to revolutionize traditional IoT systems by enhancing data security, privacy, and transparency. The scientific community has shown a variety of healthcare applications based on artificial intelligence (AI) that improve health diagnosis and monitoring practices. Moreover, technology companies and startups are revolutionizing healthcare with AI and related technologies. This study illustrates the implication of integrated technologies based on BC, IoT, and AI to meet growing healthcare challenges. This research study examines the integration of BC technology with IoT and analyzes the advancements of these innovative paradigms in the healthcare sector. In addition, our research study presents a detailed survey on enabling technologies for the futuristic, intelligent, and secure internet of health things (IoHT). Furthermore, this study comprehensively studies the peculiarities of the IoHT environment and the security, performance, and progression of the enabling technologies. First, the research gaps are identified by mapping security and performance benefits inferred by the BC technologies. Secondly, practical issues related to the integration process of BC and IoT devices are discussed. Third, the healthcare applications integrating IoT, BC, and ML in healthcare environments are discussed. Finally, the research gaps, future directions, and limitations of the enabling technologies are discussed.

This article surveys the literature in search of systems and components that use Blockchain or Smart Contracts to manage computational resources, store data, and execute services using the Cloud paradigm. This paradigm has extended from warehouse-scale data centres to the edge of the network and in between, giving rise to the domains of Edge and Fog Computing. The Cloud Continuum encompasses the three fields and focuses on the management of applications composed of connected services that span from one end to the other of the computational spectrum. Several components that are commanded by Smart Contracts are identified and compared concerning their functionality. Two important research directions are the experimental evaluation of the identified platforms and the identification of standards that can accelerate the adoption of Blockchain-based Fog platforms.

This study explores integrating blockchain technology into the Internet of Medical Things (IoMT) to address security and privacy challenges. Blockchain’s transparency, confidentiality, and decentralization offer significant potential benefits in the healthcare domain. The research examines various blockchain components, layers, and protocols, highlighting their role in IoMT. It also explores IoMT applications, security challenges, and methods for integrating blockchain to enhance security. Blockchain integration can be vital in securing and managing this data while preserving patient privacy. It also opens up new possibilities in healthcare, medical research, and data management. The results provide a practical approach to handling a large amount of data from IoMT devices. This strategy makes effective use of data resource fragmentation and encryption techniques. It is essential to have well-defined standards and norms, especially in the healthcare sector, where upholding safety and protecting the confidentiality of information are critical. These results illustrate that it is essential to follow standards like HIPAA, and blockchain technology can help ensure these criteria are met. Furthermore, the study explores the potential benefits of blockchain technology for enhancing inter-system communication in the healthcare industry while maintaining patient privacy protection. The results highlight the effectiveness of blockchain’s consistency and cryptographic techniques in combining identity management and healthcare data protection, protecting patient privacy and data integrity. Blockchain is an unchangeable distributed ledger system. In short, the paper provides important insights into how blockchain technology may transform the healthcare industry by effectively addressing significant challenges and generating legal, safe, and interoperable solutions. Researchers, doctors, and graduate students are the audience for our paper.
