Secretory function of the stomach of Japanese with endoscopically normal gastric mucosa

Abstract: Using gastrin-like AOC-tetrapeptide the secretory function of the stomach of Japanese with endoscopically normal gastric mucosa was examined in a multicenter clinical study. Mean one hour secretion volume, mean maximal acidity, and mean maximal acid output were found to be 129.3 +/- 60.0 ml/hr, 103.7 +/- 32.4 mEq/l, and 9.2 +/- 6.9 mEq/hr respectively. Results were analysed according to sex or age by analysis of variance and compared further with the corresponding data reported in the literature.

“…Among the H. pylorinegative subjects in this study, there was a considerable difference in acid secretory capacity between men and women; acid secretion in women is 56% of that in men. The difference in acid secretion according to gender has been reported in a number of earlier papers, before the recognition of H. pylori, on the study of Western control subjects (21)(22)(23) and Japanese subjects (16). In this study population, there was a substantial difference in mean body weight between men and women (66 AE 10 kg, 54 AE 9 kg, respectively).…”

“…Although, in H. pylori eradication regimens, the licensed oral dosage is 20 mg twice daily as part of a triple therapy regimen, several studies have suggested that the optimum dose of rabeprazole in H. pylori eradication regimens is only 10 mg twice daily in Asians, 29 , 30 , 36 and that higher dosages appear to offer no improvement in eradication rates. Nevertheless, acid secretion in the Japanese population is considered to be less copious than that in Western populations partly because of genetic differences and the high prevalence of atrophic gastritis 70 …”

“…The hypothesis brought to the suggestion of using 20 mg/day of rabeprazole in a Japanese population 10 is that acid secretion in the Japanese is less copious than that in a western population. 20 Unfortunately, no data for acid secretion were available for a Thai population. The adverse effects in each of the three treatment groups were high, but intolerable adverse effects (leading to early discontinuation of treatment) were documented in only two patients, both of whom were in the omeprazole group.…”
