Background: Malignant melanomas mimicking seborrheic keratosis (SK-like MMs) carry the risk of delayed diagnosis and inadequate treatment. The value of dermoscopy to improve the correct detection of these mimickers has not been previously studied. Objective: To evaluate the diagnostic accuracy of clinically SK-like MMs with and without dermoscopy. Methods: Clinical and dermoscopic images of histopathologically proven SK-like MMs (n = 134) intermingled with other melanomas and benign tumors were randomly presented to clinicians with different levels of experience, blinded to the diagnosis and goal of the study. Each participant classified each lesion as melanoma or benign tumor. The clinical and clinical-dermoscopic diagnostic accuracies were measured separately. Results: Overall, 54 participants with a mean clinical experience of 15.8 years (SD 11.8) evaluated 231 tumors. Almost 40% of SK-like melanomas were clinically misclassified as benign tumor. Introducción: la queratosis seborreica es un tumor epidérmico común, no melanocítico, benigno, que se presenta alrededor de los 30 años, que no requiere confirmación histopatológica para realizar su diagnóstico. Sin embargo, en ocasiones, las características clínicas y dermatoscópicas no son específicas, lo que dificulta su diferenciación de un melanoma, neoplasia cutánea con alta tasa de mortalidad. Caso clínico: en este artículo se presenta el caso de un paciente que consultó por una lesión con apariencia clínica de queratosis seborreica y el reporte histopatológico lo definió como melanoma, y en contraste, se presenta otro paciente con lesión clínica sugestiva de melanoma cuya patología reportó queratosis seborreica. Conclusión: al tener un alto índice de sospecha de melanoma, resulta indispensable realizar un estudio histopatológico y llevar a cabo una correcta correlación clínica-dermatoscópica-histopatológica.

Giant seborrheic keratosis of the face – an unusual presentation

Seborrheic keratosis is the most common benign epidermal lesion in the world, especially among the elderly. Its inherent benign nature has precluded the need to remove it for medical reasons. Most of the concerns presented to dermatologists or plastic surgeons are of cosmetic reasons, besides some unusual appearances that necessitate cutaneous malignancy evaluation. Unusually large sizes of seborrheic keratosis are rarely reported, and its clinical significance is largely unknown. It has been proven by recent molecular studies that seborrheic keratosis is true neoplasia rather than a mere epidermal hyperplasia, and various authors have reported several cases of concomitant malignancy arising from seborrheic keratosis. Plastic surgeon expertise is often required when faced with an extensive lesion, requiring reconstructive procedures to preserve good aesthetic and functional outcomes. The purpose of this review is to report a case of an unusually large seborrheic keratosis on the face, highlighting its clinical relevance and surgical management.
