Objective: To evaluate the nutritional status of vitamin D in urban populations of healthy elderly people living at home, in different regions of Argentina. Design: Cross-sectional study. Subjects: In total, 386 ambulatory subjects over 65 y of age from seven cities (between latitude 261S and 551S) were asked to participate between the end of winter and the beginning of spring. Of these, 369 accepted, 30 were excluded because of medical history or abnormal biochemical determinations. Finally, 339 subjects (226 women and 113 men) (X7s.d.) (71.37 5.2 y) were included. Results: Serum 25OHD levels were lowest in the South (latitude range: 411S-551S): 14.275.6 ng/ml (Po0.0001vs North and Mid regions); highest in the North (261S-271S): 20.777.4 ng/ml (Po0.03 vs Mid, Po0.0001vs South); and intermediate in the Mid region (331S-341S) 17.978.2 ng/ml. Serum mid-molecule PTH (mmPTH) and 25OHD were inversely related: (r ¼ À0.24, Po0.001). A cutoff level of 25OHD at which serum mmPTH levels began to increase was established at 27 ng/ml. A high prevalence (87-52%) of subjects with 25OHD levels in the deficiency-insufficiency range (25OHD levels o20 ng/ml) was detected. Conclusion: This study shows that vitamin D deficiency/insufficiency in the elderly is a worldwide problem. Correction of this deficit would have a positive impact on bone health of elderly people. ABSTRACT. Background. Vitamin D is essential for skeletal growth, but there are currently no guidelines for vitamin D supplementation after infancy. This study investigates vitamin D insufficiency in healthy children.Methods. Children ages 10 to 16 years from 3 private schools in Beirut, Lebanon, with differing socioeconomic status (SES) were studied: 169 in the spring of 1999 and 177 in the following fall; 83 students participated in both study phases. They had a physical examination, answered a dietary questionnaire, and blood was drawn for calciotropic hormones and indices of bone turnover.Results. Overall, 52% of the students were vitamin D-insufficient; the proportion of insufficiency was 65% in the winter and 40% at the end of the summer. During both seasons, girls had lower vitamin D levels than did boys; those who followed the dress code of covered head, arms, and legs had the lowest levels. Students in the mid-SES school had lower 25-hydroxyvitamin D (25-OHD) levels than did the ones from the high-SES school. After adjusting for confounders, gender, SES, and body mass index remained the significant predictors of vitamin D levels in both seasons (R 2 ‫؍‬ 0.53, for spring and 0.28 for fall). There was a significant inverse correlation between 25-OHD levels and parathyroid hormone levels that was best fitted by a curvilinear model (R 2 ‫؍‬ 0.19). S e ha reportado un notable incremento en investigaciones que sugieren el papel transcendental de la vitamina D en las funciones metabólicas del cuerpo humano. Está muy bien estudiado su rol primario en el mantenimiento de la homeostasia del calcio y del fósforo, bien sea por incremento en la absorción de calcio intestinal o por su actividad en el metabolismo óseo 1 .Sin embargo, la vitamina D también ejerce efectos sobre el sistema inmunológico, microendocrino de la vasculatura y en la prevención de distintos tipos de cáncer 2 . Sus acciones a nivel del sistema cardiovascular están fundamentadas en estudios que sugieren la ingesta de vitamina D para prevenir patologías, tales como la aterosclerosis 3 , hipertensión 4 , resistencia a la insulina 5 e hiperglicemia 6 ; factores de riesgos esenciales en la aparición del síndrome metabólico. El presente es un artículo de revisión que tiene como objetivo valorar las acciones no clásicas de la vitamina D que están involucradas en la génesis de esta condición clínica que eleva el riesgo de sufrir un evento cardiovascular. Vitamina D: Síntesis, metabolismo y funciones biológicas clásicasLa vitamina D 3 o colecalciferol proviene del 7-dehidrocolesterol o provitamina D 3 , compuesto producido en grandes cantidades en la piel de muchos animales vertebrados, incluyendo los humanos. Durante la exposición a los rayos solares, el 7-dehidrocolesterol absorbe la radiación ultravioleta B (UVB), con una longitud de onda entre 290-315 nm.
