Screening and early detection of pancreatic cancer in high risk population

Abstract: Pancreatic cancer is a serious growing health issue in developed countries. For patients diagnosed with pancreatic cancer, the five year survival rate is below 5%. One major important reason leads to the poor survival rate is lack of early detection of pancreatic cancer. Over 80% of the patients are diagnosed in advanced disease stages. Screening for pancreatic cancer is a desirable option for high risk individuals to allow early detection and treatment of curable pancreatic neoplasms at a pre-invasive stage. … Show more

“…However, the radiation risk and contrast agent exposure during CT examination makes this modality not suitable for routine screening and surveillance. The long durability of MRI examination time and the cost also become debatable matters when using this modality for early detection and screening in general population 9, 10, 11…”

“…It is considered to be amongst the 10 most common tumors affecting the population of the United States, with the number of expected deaths in 2015 equaling approximately 80% of the estimated new cases (1) . One of the main reasons for such a high death rate and unfavorable prognosis is related to late diagnosis; the first symptoms appear after approximately a decade of pancreatic carcinogenesis, when the tumor has reached an advanced stage 2 , 3 . As a consequence, only 20% of pancreatic cancer patients are candidates for surgery, (4) as the majority of pancreatic cancer patients show locally advanced or metastatic disease.…”

“…It remains a concern that screening for pancreatic cancer could do more harm than any potential benefit. Many clinical risk factors associated with pancreatic cancer have been identified as age, obesity, smoking, diabetes, and chronic pancreatitis; however, the specificity of these factors to pancreatic cancer is low [15].…”

“…International cancer of the pancreas screening (CAPS) has proposed a definition of ''successful screening", which is the detection and treatment of T1N0M0 margin negative pancreatic cancer and high grade dysplasia, including intraepithelial pancreatic neoplasia-3 (PanIN-3), intraductal papillary mucinous neoplasm (IPMN) with high grade dysplasia, and mucinous cystic neoplasm with high grade dysplasia [15]. Pancreatic cancer has been identified to be a rapidly progressing pathology.…”
