MRI‐based abnormalities in young adults at risk of adverse bone health due to childhood‐onset metabolic & endocrine conditions

Abstract: MRI provides valuable detailed information on the micro-architecture and adiposity of bones and is capable of showing clear differences in bone parameters in a range of clinical conditions associated with abnormal bone health.

“…Increased trabecular spacing is not universally associated with increased bone marrow adiposity . Although this association may be encountered in GH deficiency, in other conditions such as osteogenesis imperfecta, the two parameters do not show an association . It is also possible that in states of GH resistance such as T1DM, bone marrow adiposity may be increased.…”

“…1H‐MRS was performed using a 6‐channel body array (anterior) and a 12‐channel spine coil (posterior). Spectra were obtained from a 20‐mm × 20‐mm × 20‐mm volume within the vertebral body of L 3 , using a method that has been described previously . Analysis was performed following fitting of the spectrum in the time domain using a nonlinear least‐squares algorithm, AMARES, in the Java‐based magnetic resonance user interface (jMRUI) software package .…”

“…There may be a place to incorporate evaluation with bone histomorphometry as a form of monitoring in those treated with bisphosphonate to guide duration of therapy. Newer modalities of bone imaging including high-resolution pQCT and micro-MRI [30,46] may allow assessment of bone microarchitecture, and these findings may closely correlate with bone biopsy results [47], although such imaging modalities are currently only available as research tools. …”

“…Thorough, in vivo assessment of adiposity at multiple sites, including abdominal, total body and bone marrow, requires several methods of imaging and hitherto this has rarely been performed in humans . There is an increasing realization that a comprehensive assessment of bone requires an assessment of bone microarchitecture as well as bone densitometry . Metabolic conditions that may affect bone health, such as diabetes, are also associated with marked alterations in body composition, adiposity and BMA .…”
