Purpose To describe the anatomic characteristics of the lacrimal sac and its adjacent bone structures and to provide surgical recommendations for endoscopic dacryocystorhinostomy (EN - DCR) in Asians. Methods This retrospective comparative study involved 118 sides with complete nasolacrimal duct obstruction and 83 unaffected sides. Computed tomographic - dacryocystography (CT-DCG) scans were performed before lacrimal surgery, and image reconstruction was used to obtain continuous 0.75-mm axial, coronal, and segmental sections for review. The morphology of the lacrimal sac and its relationship with adjacent bony structures were determined for measurement, as well as the obstructed location. Results The height of the lacrimal sac was 12.99 ± 2.10 mm in this study. The operculum of the middle turbinate (OMT) was located vertically in the lower third of the lacrimal sac. Horizontally, the junction between the maxillary bone and the lacrimal bone (MB-LB) was close to, mostly (60.2%) posterior to, the lacrimal sac. The uncinate process was more frequently attached to the lacrimal bones (75.1%). The obstructions were generally located around the entrance of the nasolacrimal duct (NLD); however, some were placed higher, with 7.63% blocked not lower than the OMT. There was a negative correlation between the diameter of the lacrimal sac and the level of obstruction (r= -0.35, p < 0.01). Conclusion In this study, the OMT and MB-LB can be served as the landmarks in EN - DCR. Partial uncinectomy should be performed in most cases. The obstructions were generally located around the entrance of the NLD, but some extreme individual variations strongly implies the importance of CT-DCG scanning before surgery.

Objectives: Obstructions are very commonly seen in nasolacrimal duct before it opens into the inferior nasal meatus. Detailed anatomical knowledge of the nasolacrimal duct is crucial for physicians to understand the etiology of the obstructions, to plan ideal management option and to reduce unexpected iatrogenic injuries during surgeries. The aim of this study was to investigate morphometric properties of the nasolacrimal duct on computed tomography images. Methods: Three-dimensional computed tomography (3D-CT) of 142 adults (65 females, 77 males) were retrospectively evaluated. Antero-posterior cranial diameter, antero-posterior and transverse diameters and vertical angle of the nasolacrimal duct, distance between distal end of the nasolacrimal duct to anterior surface of the maxilla were measured and the differences evaluated statistically between right and left sides and among females and males and among different ages. All measurements were done using Osirix-Lite version 9 software. Results: None of the morphometric parameters of the nasolacrimal duct showed significant differences between right and left sides. Antero-posterior cranium and transverse diameter of the nasolacrimal duct were longer in men than women. Conclusion: Determining to morphometric properties of the nasolacrimal canal has advantages for understanding the etiology of the NLD obstructions, deciding the ideal surgical technique and reducing to unexpected injuries during surgeries related with this region.

Objective: The intent of this chapter is to provide a systematic reappraisal of the bony anatomy of the orbit. The studies and the available literature on this topic are ever-expanding. The present knowledge shall be put into perspective.Material and Methods: Textbook and atlas descriptions served as a starting point for a comprehensive outline of the osseous anatomy of the orbits. To deepen the insight and understanding, a small selection of recent reports on specific structures has been reviewed. For visual purposes, photographs of PMHS (post mortem human subject) specimens and illustrations were used.Review/Results: The most distinctive feature of each orbit is the quadrangular pyramidal shape with a tetrahedron spire making up the orbital apex. The bony elements of the orbits are delineated with the sphenoid as backward foundation and starting base for the overall architectural composition. A closer look goes into the openings within the orbital precincts and reveals the subtleties and variations of the fissures, canals, grooves, foramina, notches, and fossae monitoring recent publications. The research modalities of these studies have shifted from dry skull investigations and PMHS dissections to modern imaging techniques (CT, MRI, CBCT) within defined living populations facilitating the analysis of hidden spaces or hard-to-reach structures in a non-destructive manner. Additional advantages of imaging include quantification and a detailed morphometric evaluation in the spirit of computational anatomy.Conclusion: In essence, the combination of traditional anatomic knowledge and the understanding of the tremendous complexity of variations characterized in new publications demands for a patient-specific diagnostic workup as exemplified in this book. As a matter of fact, however it is more likely to recognize and grasp features that one knows or knows about from previous intensive learning.
