Sciatic Nerve Schwannoma

Abstract: Schwannomas are benign tumors of the nerve sheath, commonly seen in the vestibular nerve. Although they are common in the head and neck areas, their occurrence in extra-cranial locations is rare. We report a case of a sciatic nerve schwannoma and review the literature.

“…Since the symptoms are similar to sciatica due to a herniated disk, while imaging of the spine may reveal discopathy, intrapelvic MPNST of the sciatic nerve is easily misdiagnosed (8,9). A misinterpretation of the findings may result in incorrect treatment by discectomy, as reported in our patient.…”

“…2,10,12,16 Surgical excision is the treatment of choice. Schwannomas are theoretically removable since they repulse fascicular groups without penetrating them thus allowing their enucleation while preserving nerve continuity, 8,11,14,15 as reported in our patient. Microsurgical excision should be performed using electrical stimulation to facilitate detection of motor fascicles.…”

“…The most common clinical presentation of sciatic nerve schwannoma consists of a painful palpable mass. 9,11,13 Schwannomas most commonly occur in adults between 20 and 50 years of age. Schwannomas are usually homogeneous on both T1-and T2-weighted images but a neurofibroma is usually heterogeneous.…”

“…Malignant schwannoma constitute 2% of the nerve sheath tumors and they have a predilection for larger diameter peripheral nerves such as brachial plexus or Sciatic nerve. Malignant transformation of benign schwannomas is unusual [16].The slow growth pattern of the tumor allows for the adaptation of the nerve function to the pressure effects which leads to the delayed presentation and diagnosis [17].Schwannomas of the median nerve are rare tumors. They occur commonly in the distal region of the upper extremity (at elbow or distal), occurs more in the mixed nerves. Two types of presentation are seen.…”
