Schema therapy for emotional dysregulation in personality disorders

Abstract: Schema therapy is one of the most efficient therapies for personality disorders. However, there is a lack of recent studies on how it treats emotion dysregulation. Although the treatment of emotional dysregulation is not the core of schema therapy, it is certainly important inside this theoretical framework. The mode model helps clinicians address their work toward the reduction of dysfunctional modes, whereas fostering functional modes.

“…The use of experiential techniques have been found to distinguish ST from CBT and psychodynamic therapies [ 12 ]. Experiential techniques including imagery rescripting and chair work dialogues have been linked to improving emotional dysregulation in personality disorders [ 13 ]. “…On the other hand, the use of chair work allows for dialogues and interactions with significant persons in the re-enactment of a past situation, and is particularly useful in increasing the metacognitive capacity of individuals when working with their respective modes [ 15 ]. Both experiential techniques serve as agents in facilitating one’s access to emotions and unmet core needs [ 13 ].…”

“…However, other authors claim that there is no need to increase the number of modes. In fact, for some applications of ST (e.g., couple schema therapy), some authors claim that a simplification of the model is necessary (Dadomo et al 2018;Simeone-DiFrancesco et al 2015). To understand a patient who shows severe emotional dysregulation, using the concept of modes is essential.…”

“…To understand a patient who shows severe emotional dysregulation, using the concept of modes is essential. The mode theory's basic concept shows that different mental states have different purposes and are related to different basic needs (Dadomo et al 2018). In addition, if the therapy itself works directly with one active mode at a given moment, it follows that it is very important to be able to identify it precisely.…”

“…In daily life, the activation of dysfunctional modes negatively influences parents' attitudes and behaviors toward their child. This is because DCM and DPM are associated with either (1) dysregulated emotions, or with (2) dysregulatory mechanisms (Dadomo et al, 2016 , 2018 ). DPM are the primary source of dysregulated emotions and reflect pathological aspects of the parent that are enacted inside the relationship.…”
