Scan findings of various myocardial SPECT agents in a case of amyloid polyneuropathy with suspected myocardial involvement

Abstract: A 31-year-old male having familial amyloid polyneuropathy underwent a Tc-99m(V)-DMSA study to evaluate the myocardial involvement. The patient also underwent T1-201, I-123-BMIPP and I-123-MIBG myocardial SPECT studies to evaluate blood perfusion, fatty acid metabolism and sympathetic function of the heart, respectively. Tc-99m(V)-DMSA SPECT showed uptake to the myocardium indicating myocardial involvement of amyloidosis. Both T1-201 and I-123-BMIPP studies showed normal uptake indicating normal blood perfusion… Show more

“…This was later confirmed by others 33-35 indicating that patients with FAP have a high incidence of myocardial adrenergic denervation with viable myocardium and can be identified early in cardiac amyloidosis before clinical heart disease and echocardiographic changes. 34 Another clinical trial by Delahaye et al involving 17 patients with rectal or nerve biopsy-proven FAP found that cardiac 123 I-MIBG uptake was significantly decreased in FAP patients with no difference in washout rates despite preserved left ventricle systolic function and cardiac perfusion.…”

“…The SPECT images showed abnormal uptake of the tracer in the heart. They suggested that in this patient either 99m Tc-DMSA or 123 I-MIBG may be useful to detect involvement of the heart by the amyloid process [12].…”

“…99m Tc (V)-DMSA showed some myocardial involvement of amyloidosis, and 201 Tl and 123 I-BMIPP showed normal blood perfusion and normal fatty acid metabolism, respectively (all these tracers are discussed below in this review). The decreased uptake of MIBG in the heart indicated a severe impairment of sympathetic function [12].…”

“…Its uptake in AL amyloidosis was described by Ohta et al [31] and Kobayashi et al [22]. Disadvantage of 99m Tc-penta-DMSA is its relatively high blood-pool concentration making the assessment of tissue uptake difficult, which was minimized by delayed image registration after 24h post injection and described by Arbab et al [32]. …”

“…The isotope used was 131 I and β2M was extracted from uremic hemofiltrate. The radiotracer is accumulated directly in amyloid deposits [32]. A disadvantage of 131 I-labeled radiotracer was poor spatial resolution, which made imaging of small joints problematic and increased the radiation dose due to unnecessary β(−) radiation [34].…”
