The aim of our study was to evaluate results of insertion following cochlear implantation with Contour™ and Contour Advance™ electrode arrays in adult patients and to analyze individual insertion results for three experienced surgeons. We performed a retrospective analysis of postoperative 3D volume tomography results in 223 adult patients. The intracochlear electrode position was evaluated to be in scala tympani, scala vestibuli or with a dislocation from one scala to the other. Surgical methods were analyzed and assigned to the different surgeons. We observed a significant increase for scala tympani insertions from initially 33% to 84% and a reduction in dislocations from scala tympani to scala vestibuli from 71% with the Contour™ electrode to 22% with the Contour Advance™ electrode. Results for the different surgeons varied individually with regard to scala tympani insertion rates and dislocation rates over time. 3D Volume tomography offers an important method for postoperative quality control following cochlear implant surgery. The intracochlear electrode position could be determined in all cases. We were able to identify individual learning curves for insertion results. Controlling the insertion quality serves as a feedback of surgical results and may be helpful for improving surgical quality and thus rehabilitation results.

Während in Einzelfällen bei Elektrodeninsertion in die Scala vestibuli gute postoperative Hörergebnisse berichtet werden [ 230 ] , konnte in einer klinischen Studie an 43 Patienten gezeigt werden, dass das ein Jahr nach Cochlea-Implantation gemessene Sprachverstehen in Abhängigkeit von der Elektrodenposition in folgender Reihenfolge abnahm: Scala tympani > Dislokation von Scala tympani in Scala vestibuli > Scala vestibuli [ 231 ] . Eine weitere Untersuchung ergab, dass die Performanz der CI-Träger nach 6 Monaten positiv mit der Elektrodenzahl in der Scala tympani korreliert [ 232 ] .…”

Surgery of the ear and the lateral skull base is a fascinating, yet challenging field in otorhinolaryngology. A thorough knowledge of the associated complications and pitfalls is indispensable for the surgeon, not only to provide the best possible care to his patients, but also to further improve his surgical skills.Following a summary about general aspects in pre-, intra- and postoperative care of patients with disorders of the ear/lateral skull base, this article covers the most common pitfalls and complications in stapes surgery, cochlear implantation, surgery of vestibular schwannomas, and jugulotympanal paragangliomas. Based on these exemplary procedures, basic "do's and don'ts" of skull base surgery are explained, which the reader can easily transfer to other disorders. Special emphasis is laid on functional aspects, such as hearing, balance and facial nerve function. Furthermore, the topics of infection, bleeding, skull base defects, quality of life and indication for revision surgery are discussed.An open communication about complications and pitfalls in ear/lateral skull base surgery among surgeons is a prerequisite for the further advancement of this fascinating field in ENT surgery. This article is meant to be a contribution to this process.

Abstract. In previous works, the authors showed that using robot-assisted steerable electrode array insertions can significantly reduce the insertion forces compared to non-steerable electrode arrays. In addition to steering the electrode array, it is possible to change its angle of approach with respect to the scala tympani. This paper focuses on determining the relevance of changing the angle of approach of the electrode array by comparing steerable electrode array insertions using a two Degreesof-Freedom (DoF) robot versus a four DoF robot. Optimal insertion path planning strategies are presented for both two and four DoF insertions. Simulation results and experiments show that the four DoF insertions can improve over two DoF insertions. Moreover, changing the angle of approach can further reduce the insertion forces. The simulation results also provide the workspace requirements for designing a custom parallel robot for robot-assisted cochlear implant surgery.
