Asymmetry, especially in the midface, appears to detract from how facial appearance is self-perceived and perceived by others, which explains the primary desire for or need of nose correction. The self-perception of patients affected by CLP does not correlate with objective results or how others perceive them. Clinicians should be open to adult patients' requests for correction, but the patient's self-perception should also be critically explored.

Cleft lip and palate represent a major public health problem due to the possible associated life-long morbidity, complex etiology, and the extensive multidisciplinary commitment required for intervention. It affects about 1.5 per 1000 live births (250,000 new cases per year) worldwide, with tremendous variations across geographic areas and ethnic groups. It is considered a debilitating condition that is associated with significant feeding, hearing, speech, and psychological impairments. The wide surgical, dental, speech, social, and medical involvement emphasize the importance of understanding the underlying determinants of these defects to allow optimizing the treatment options and predicting the long-term course of the affected individuals development. Optimal and early surgical intervention is necessary and folic acid supplementation proved to be a highly efficient preventive strategy. However, there are still many challenges to be addressed for cleft care especially in the developing parts of the world.

O presente estudo avaliou as disfunções orofaciais em pacientes com e sem fissuras labiopalatinas e correlacionou com qualidade de vida. Foram selecionados 120 pacientes, de 35 a 65 anos do Hospital de Reabilitação de Anomalias Craniofaciais/USP e da Faculdade de Odontologia de Bauru FOB/USP. Os pacientes selecionados foram divididos em 2 grupos: grupo controle (GC) e grupo fissura (GF): 60 pacientes em cada grupo (30 homens e 30 mulheres). Cada paciente passou por uma entrevista e por uma avaliação clínica, através dos instrumentos NOT-S (disfunção orofacial) e SF-36 (saúde relacionada a qualidade de vida). A pontuação foi obtida e tabulada para cada instrumento específico. O teste estatístico de MannWhitney foi aplicado para avaliar grupos e gêneros no desempenho de NOT-S e SF-36 e o Teste Qui-quadrado associou o desempenho entre NOT-S, nos gêneros e grupos. Para correlacionar os instrumentos utilizou-se o Coeficiente de correlação de Spearman. Nível de significância de 5% foi adotado para todos os testes estatísticos. O índice de concordância intra-examinador foi K=0,63. Ao avaliar os valores de NOT-S entre os grupos, houve diferença estatisticamente significativa para os gêneros (p<0,001), entretanto quando avaliou-se os gêneros no mesmo grupo não houve diferença. Na avaliação do desempenho dos domínios de NOT-S entre GC e GF, houve diferença estatística ao associar os grupos nos domínios II (p=0,021), IV (p<0,001) e VI (p=0,002) na entrevista, e em todos os domínios do exame (p<0,001). Na avaliação dos domínios do SF-36 com os grupos observou-se médias de valores maiores em todos os domínios, com diferença estatística para: estado geral de saúde (p=0,003), aspectos físicos (p=0,014), aspectos sociais (p<0,001), e vitalidade (p=0,006). Ao associar o questionário SF-36 entre os gêneros e os grupos houve diferença estatística para o gênero feminino nos seguintes domínios: estado geral de saúde (p=0,002), aspectos sociais (p=0,003) e vitalida...
