Sarcomatoid salivary duct carcinoma of the oral cavity

Abstract: This report recounts a new case of sarcomatoid variant of salivary duct carcinoma in a 45-year-old woman. The 1.5-cm polypoid mass protruded from the retromolar area and focally extended into the pharyngeal wall. The patient was free of disease for 11 years. The tumor showed a biphasic malignancy; nondescript sarcomatous proliferation predominated over ductal carcinoma population, with perceptible blending of the two components. The sarcomatoid areas had no heterologous properties. Discohesive pleomorphic cell… Show more

“…Dedifferentiation in the form of high-grade sarcomatoid tumor has been documented in nearly every type of salivary gland carcinomas [3,8,[10][11][12][13]. Our case must be distinguished cautiously from dedifferentiated MEC [7,14].…”

“…A carcinomatous component shows the cytomorphologic aspect of conventional SDC with cribriform or solid cell nests, ductal carcinoma in situ, and comedonecrosis, whereas a sarcomatoid component is composed of mitotically active, pleomorphic spindle cells and anaplastic tumor giant cells. [1][2][3][4][5] In the present case, the histologic findings of sarcomatoid SDC described above were detected. Furthermore, in the present case, a squamous cell carcinoma component was also found.…”

“…There have been dozens of reports of histologic findings of sarcomatoid SDC. [1][2][3][4][5] Histologically, sarcomatoid SDC is characterized by the presence of both a carcinomatous and a sarcomatoid component. A carcinomatous component shows the cytomorphologic aspect of conventional SDC with cribriform or solid cell nests, ductal carcinoma in situ, and comedonecrosis, whereas a sarcomatoid component is composed of mitotically active, pleomorphic spindle cells and anaplastic tumor giant cells.…”

“…Dedifferentiation in salivary gland carcinomas other than ADCC is a rare but well recognized phenomenon and has been documented in epithelial-myoepithelial carcinoma (EMECa) [34][35][36][37][38], acinic cell carcinoma [39][40][41][42][43][44], mucoepidermoid carcinoma [45,46], polymorphous low-grade adenocarcinoma [47,48], hyalinizing clear cell carcinoma [49] and salivary duct carcinoma [50][51][52][53]. In the dedifferentiated (recurrent) acinic cell carcinoma presented by Piana et al [42], there was evidence of myoepithelial differentiation (clear cell morphology, widespread positivity for S100 protein and focal immunoreactivity for SMA).…”
