Safety and diagnostic accuracy of percutaneous biopsy in upper tract urothelial carcinoma

Abstract: upper tract lesions in 24 patients (20 men; median [range] age 67.8 [51.7-85.9] years) were percutaneously biopsied. Analysis was separated based on lesion appearance: (i) mass infiltrating renal parenchyma, (ii) filling defect in the collecting system, (iii) urothelial wall thickening. We tracked immediate complications and tract seeding on follow-up imaging. . Biopsy characterised 14 of 15 infiltrative masses and five of five filling defects; biopsy characterised three of six cases of urothelial wall thicke… Show more

“…Furukawa et al (23), assessed a total of 40 patients and found that ureteroscopic biopsy could lead to determination of the pathological grade of nephroureterectomy specimens with an accuracy rate of 87.5%. Recently, Huang et al (24) reviewed 26 cases of upper tract lesions and found that percutaneous biopsy was also a safe and effective method.…”

“…2 Furthermore, percutaneous NT insertion to relieve obstructive uropathy in patients with renal TCC who subsequently underwent nephroureterectomy led to recurrence within the NT tract in three patients. Finally, there have only been two reported cases of tract seeding after fine needle aspiration of renal TCC with 22- and 20-gauge needles.…”

“…Huang et al reported 26 biopsies for suspected UTUC with no incident cases of tract seeding on median followup of 13 months. 16 Th0e oncologic safety of PCNB for UTUC in these 2 contemporary series may be related to improved biopsy technique and incorporation of coaxial needle sets. The majority (76.2%) of patients in our series underwent coaxial biopsy, which has been associated with the extremely low (0.01%) rate of tract seeding in contemporary series of PCNB for RCC.…”

“…Another prior series of 26 PCNBs similarly showed 6 minor but no major complications. 16 While PCNB is used to obtain histologic diagnosis in most malignancies, it has been historically avoided for suspected UTUC due to concerns for tract seeding. Importantly, there were no cases of tract seeding noted in our series of 42 patients with a median follow-up over 2 years.…”
