Eine 57-jährige Patientin stellte sich mit einer seit einem Monat bestehenden schmerzhaften Schwellung und Induration an der Außenseite des linken Oberschenkels vor. Sie gab an, dass die Schmerzen unter Belastung zunähmen und das Hautareal bei Berührung ebenfalls stark schmerze. Ambulant erfolgten internistische und orthopädische Untersuchungen. Ein Muskelfaserriss sowie eine tiefe Beinvenenthrombose wurden nicht nachgewiesen, die Symptomatik ließ sich jedoch nicht näher einordnen. Die Ergebnisse der auswärtigen Bestimmung von Entzündungsparametern, antinukleären Antikörpern sowie IgM und IgG gegen Borrelien waren unauffällig. Analgetische und antiphlogistische Therapie mit Etoricoxib 60 mg (2 x täglich) und Ibuprofen 400 mg (2-3 x täglich) über vier Wochen hatte keinen Einfluss auf die Beschwerden. Die Patientin litt an arterieller Hypertonie und gastroösophagealem Reflux. Sie nahm keine Medikamente.
