An 81-year-old woman on anticoagulant therapy after mechanical heart valve replacement was admitted because of acute aortic dissection. Anticoagulant therapy had to be continued with heparin even after admission. Gastrointestinal hemorrhage occurred suddenly and she developed hemorrhagic shock. Computed tomography findings suggested that the bleeding was due to rupture of a pseudoaneurysm of the pancreaticoduodenal arcade. After the site of bleeding was identified by angiography, hemorrhage was successfully controlled by embolization with coils. With the aging of the population, vascular complications of arteriosclerosis are likely to increase. This case report provides important insights that could be helpful for treating such patients.

BACKGROUNDPancreaticoduodenal artery (PDA) aneurysms are extremely rare. The complicated clinical presentations and high incidence of rupture make it difficult to diagnose and treat. PDA pseudoaneurysms often rupture into the gastrointestinal tract and result in life-threatening gastrointestinal hemorrhage.CASE SUMMARYA 69-year-old man was admitted to our hospital due to right upward abdominal pain. A computed tomography (CT) scan demonstrated acute cholecystitis and cholecystolithiasis. Percutaneous gallbladder drainage was performed subsequently. He was discharged after 3 d and readmitted to hospital for cholecystectomy as arranged 1 mo later. A repeat CT scan revealed an emerging enhancing mass between the pancreatic head and the descending duodenum. Then, he suffered hematochezia and hemorrhagic shock suddenly. Emergency percutaneous angiogram was performed and selective catheterization of the superior mesenteric artery demonstrated a pseudoaneurysm in the inferior PDA. Coil embolization was performed and his clinical condition improved quickly after embolization and blood transfusion. He underwent laparoscopic cholecystectomy and was discharged from hospital after surgery under satisfactory conditions.CONCLUSIONPDA pseudoaneurysms are uncommon. Acute haemorrhage is a severe complication of pseudoaneurysm with high mortality which clinicians should pay attention to.

ÂâåäåíèåСосудистые нарушения являются достаточно ча-стым и наиболее грозным осложнением острого не-кротического панкреатита (ОНП) и обострения хро-нического панкреатита (ХП). Они включают венозные тромбозы портальных, селезеночных и брыжеечных сосудов, диффузные кровотечения из варикозно рас-ширенных вен, ишемические некрозы с образовани-ем инфаркта селезенки и некроза желудка, тонкой и толстой кишки, аррозии артериальных сосудов с об-разованием ложных аневризм (ЛА). Аррозия сосудов с образованием ЛА висцеральных сосудов, вовлеченных в некротический процесс, является наиболее опасным осложнением. Данная категория осложнений ОНП и ХП нечастая, однако наиболее грозная и тяжелая.Впервые описал аневризму селезеночной арте-рии в 1770 году M. Beaussier при аутопсии больного, умершего от внутреннего кровотечения, которая ста-ла случайной находкой. Образование ЛА наблюдается как в острый период, так и в последующее время по-сле перенесенного ОНП на этапе развития у пациен-та ХП. Предрасполагающими факторами в генезе ЛА являются дисплазия сосудов, портальная гипертензия, спленомегалия и локальные воспалительные процес-сы. Частота возникновения ЛА при ОНП составляет 1,2-14 % [1][2][3][4][5][6][7]. Зарегистрированная заболеваемость ХП, осложненного кровотечением из псевдоаневризм, колеблется от 4 до 10 % [8,9]. Наиболее часто развива-ются псевдоаневризмы селезеночной артерии, реже -гастродуоденальной, панкреатодуоденальной и пече-ночных артерий [10].Формирование ЛА имеет два варианта. В случае ар-розии артерии в ткани поджелудочной железы размеры ЛА бывают небольшими. Этот вариант ЛА протекает, как правило, бессимптомно и диагностируется случай-но на этапах динамического обследования.Второму варианту формирования ЛА предшествует образование псевдокисты. Развитие постнекротиче-ских псевдокист осложняет течение острого панкре-атита у 5-15 % больных [3].
