Ropivacaine for Unilateral Spinal Anesthesia; Hyperbaric or Hypobaric?

Abstract: Both hyperbaric and hypobaric ropivacaine (11.25mg) provided adequate and dependable anesthesia for total knee replacement surgery, with a high level of patient and surgeon comfort. Hypobaric local anesthetic solutions provide a high level of unilateral anesthesia, with rapid recovery of both sensory and motor block, and therefore may be preferable in outpatient settings.

“…1 The reduction of local anesthetic dose (adding adjuvants like opioids) and sympathectomy can contribute to more favourable haemodynamics and theoretically better outcomes in the elderly population. 16,17 We prefer to use hypobaric spinal anesthesia, due to difficulties of changing patient positions, advantage of decrease in urinary retention and better patient and surgeon satisfaction. During hypobaric spinal anesthesia, hypotension incidence is reduced, and recovery period shortened in our study.…”

“…El artículo titulado Efecto de la rotación de la cabeza en la presión intraocular en decúbito ventral: estudio aleatorizado, y publicado recientemente en la Revista Brasileña de Anestesiología, nos llama la atención sobre la importancia de la buena práctica anestésica con el fin de prevenir una complicación grave para los pacientes que se someten a la cirugía en decúbito ventral y que es la pérdida de la visión 1 .…”

“…In unilateral spinal anesthesia, a hypobaric solution is administered into the subarachnoid space, with the patient placed in the lateral decubitus position ( 7 - 9 ). Limiting the spread of the spinal block offers many clinical advantages, including greatly reducing the hemodynamic impact of spinal anesthesia ( 10 ). In addition, unilateral spinal anesthesia can be useful in elderly patients who have low cardiac output and a risk of early postoperative embolization ( 10 ).…”
